紫杉醇脂质体加奈达铂联合放疗治疗老年食管癌的临床观察

摘　　要：目的观察紫杉醇脂质体加奈达铂诱导化疗后三维适形放疗治疗老年食管癌的疗效及毒副反应。方法 60例老年食管癌患者随机分为2组。放化组30例:先予紫杉醇脂质体加奈达铂化疗,化疗1周后开始三维适形放疗;单放组30例:单纯采用三维适形放疗。结果放化组、单放组近期有效率分别为93.3%和83.3%,差异无统计学意义(P>0.05)。2组的毒副反应以Ⅰ～Ⅱ度为主,放化组骨髓抑制发生率高于单放组(P<0.05)。结论紫杉醇脂质体加奈达铂化疗联合放疗治疗老年食管癌近期疗效确切,毒副反应可耐受。Objective To investigate the curative effect and toxicities of paclitaxel liposome plus nedaplatin chemotherapy combined with three-dimensional conformal radiotherapy in the treatment of the elderly patients with esophageal cancer. Methods Sixty patients with esophageal cancer were randomly divided into two groups （ the chemotherapy combined with radiotherapy group and the radiotherapy group）. Thirty patients of the chemotherapy combined with radiotherapy group received the paclitaxel liposome plus nedaplatin chemotherapy first, and received the three-dimensional conformal radiotherapy after a week; 30 patients of the radiotherapy group received only the three-dimensional conformal radiotherapy. Results The curative effect rate were 93.3% and 83.3% in the chemo- therapy combined with radiotherapy group and the radiotherapy group, the difference was not statistically significant （ P 〉 0.05 ）. The toxicities of two groups were grade I - 1~ , the bone marrow suppression incidence rate of the chem- otherapy combined with radiotherapy group was higher than that of the radiotherapy group （ P 〈 0.05）. Conclusion The short term effect of paclitaxel liposome plus nedaplatin combined with three-dimensional conformal radiother- apy is exact in the treatment of the elderly patients with esophageal cancer,and the toxicities is tolerated.
