护士与临床药师合作促进抗菌药物合理使用管理模式初探

摘　　要：目的探讨医院抗菌药物临床合理使用管理模式,促进住院患者抗菌药物的合理使用,体现院感科护士及临床药师在促进抗菌药物合理使用中的作用。方法院感科护士与临床药师紧密合作,利用参加医院内科三级查房机会,组成抗菌药物合理使用检查小组,发挥各自的专业优势,对使用抗菌药物的医嘱、病历记录及合理性等进行检查及干预。结果内科治疗性使用抗菌药物的微生物送检率上升至82.6%,2012年抗菌药物使用率较2010年下降了13.5个百分点,微生物送检合格率、病程记录的完整性以及使用指征的掌握、用法用量、溶媒的选择等方面有了较明显的提高。结论医院感染科护士与药剂科临床药师密切合作、专业互补的模式,可提高医院抗菌药物的临床使用合理性,体现了院感科护士及临床药师在其中的作用。OBJECTIVE To explore the management mode for clinical rational use of antimicrobial drugs in the hospital so as to promote the rational use of antimicrobial drugs and embody the role the nurses of the infection control department and the clinical pharmacists in promoting the rational use of antibiotics.METHODS The nurses in the department of hospital infection management and clinical pharmacists worked together as an inspection team.Taking advantage of the grand round,the professional ability was utilized to inspect and intervene the doctor′s orders and cases history as well as the rationalities.RESULTS The submission rate of the microorganism for the cases treated with internal medicine increased up to 82.6%;as compared with 2010,the utilization rate of antibiotics in 2012 decreased by 13.5 percent;the qualified rate of microbial submission,integrity of disease record,the grasp of medication indication,usage and dosage,and the selection of solvents were significantly improved.CONCLUSION The mode that the nurses of the infection control department and the clinical pharmacists cooperate closely to complement professional technology each other can improve the rationality of the clinical use of antibiotics,which embodies the role of the nurses and clinical pharmacists.
