静意方治疗注意缺陷多动障碍儿童99例临床观察

摘　　要：目的评价中药静意方治疗脾虚痰蒙、心肝火旺型注意缺陷多动障碍(ADHD)儿童的疗效和安全性。方法将176例ADHD患儿随机分为静意方组99例、利他林组37例、联合组40例,静意方组给予静意方配方颗粒口服,每日1剂;利他林组给予利他林片口服,从每日5～10mg开始逐渐增量,每日总量<40mg,每周5天;联合组予静意方及利他林片,用量及方法同上。各组均3个月为1个疗程,共两个疗程。观察各组治疗前及治疗后3、6个月中医证候疗效、注意力评分及Cornners父母评定量表评分,并记录不良反应。结果治疗3个月后静意方组、利他林组、联合组中医证候疗效分别为79.80%、51.35%、82.50%,治疗6个月后分别为97.98%、83.78%、100%,静意方组和联合组中医证候疗效明显高于同时间利他林组(P<0.05或P<0.01)。治疗3、6个月后各组注意力评分和Cornners父母评定量表各因子评分均较治疗前明显降低(P<0.01),并且治疗后6个月各评分低于治疗后3个月(P<0.01);静意方组和联合组治疗后6个月注意力评分和冲动评分与利他林组比较差异有统计学意义(P<0.05或P<0.01)。静意方组无明显不良反应。结论静意方治疗脾虚痰蒙、心肝火旺型ADHD儿童临床疗效肯定,能改善患儿注意力,且无明显不良反应。Objective To evaluate the efficacy and safety of Jingyi Formula （formula to calm the will） for attention deficit hyperactivity disorder （ADHD） children with spleen-deficiency phlegm-clouding and exuberant heart-liver fire syndrome.Methods Totally 176 ADHD children were randomized into the Jingyi Formula group, Ritalin group and combo therapy group, with 99, 37 and 40 in each. The Jingyi Formula group was given both Jingyi Formula granules orally, one dose daily.The Ritalin group was given Ritalin Tablets orally, with a gradual increment of daily total from 5-10mg to less than 40mg and 5 days weekly. The combo therapy group was given Jingyi Formula and Ritalin Tablets with the same dosage and method as above. One treatment course lasted for 3 months and all groups were treated for 2 courses. The TCM syndrome efficacy, attention scores and Conners＇ Parent Rating Scale （CPRS） scores were observed before treatment and after 3-month or 6-month treatment. The adverse reaction was recorded. Results The TCM syndrome efficacy in the Jingyi Formula group, Ritalin group and combo therapy group was 79.80%, 51.35% and 82.50% after 3-month treatment, while 97.98%, 83.78% and 100 % after 6-month treatment. The TCM syndrome efficacy in the Jingyi Formula group and combo therapy group was significantly higher than that in the Ritalin group at the same time point （P〈0.05 or P〈0.01）. The attention scores and CPRS scores were significantly decreased in all groups after 3-month or 6-month treatment （P〈0.01）, especially after 6-month treatment （P〈0.01）. Comparing with the Ritalin group, the differences of attention scores and impulsiveness scores after 6-month treatment were statistically significant in the Jingyi Formula group and combo therapy group （P〈0.05 or P〈0.01）. There was no adverse reaction in all groups. Conclusion Jingyi Formula is effective for ADHD children with spleen-deficiency phlegm-clouding and exuberant heart-liver fire syndrome and it can improve the attention with no adve
