摘 要:目的采用千伏级锥形束CT(CBCT)对头颈部肿瘤调强放疗(IMRT)首次摆位进行探讨。方法对接受图像引导放疗(IGRT)40例头颈部肿瘤患者,首次摆位前进行CBCT扫描,获得X轴(左右)、Y轴(头脚)、Z轴(腹背)的摆位误差共120个数据。结果 X轴误差(1.125±0.99)mm,Y轴误差(1.4±1.16)mm,Z轴误差(1.32±1.14)mm。X轴的摆位误差最大值为2 mm,Y轴和Z轴的摆位误差最大值为3 mm;对误差>2 mm给予在线纠正。结论 CBCT可减少头颈部肿瘤首次摆位误差,并有助于确定合适的CTV(临床靶区)外放边界。Objective To investigate first setup in intensity modulated radiotherapy (IMRT) for head and neck tumor by kV level cone beam CT (CBCT). Methods There are 40 patients with head and neck tumor accepting image guide radiation therapy (IGRT). Before first setup, 120 errors are obtained from X-axis (left-right), Y-axis (head-foot), Z-axis (ventral-dorsal) by CBCT. Results The error of X-axis is (1.125±0.99) mm, the error of Y-axis is (1.4±1.16) mm, the error of Z-axis is (1.32±1.14) mm. The maximum error of X-axis is 2 mm, the maximum error of Y- axis and Z-axis is 3 mm. Errors exceeding 2 mm are verifies on-line. Conclusion CBCT can reduce first setup errors of patients with head and neck tumor and confirm the required margins for CTV (Clinical Target Volume).
