摘  要:目的比较不同吸氧浓度对改善二氧化碳气腹腹腔镜肾癌根治术后颈肩部疼痛的影响。方法 60例肾癌根治术患者,随机分为2L/min组(A组),4L/min组(B组),6L/min组(C组),8L/min组(D组)。在腹腔镜肾癌根治术后持续吸氧32h,检测4组间腹腔镜肾癌根治术后2h、4h、8h、16h和32h颈肩部疼痛发生率和疼痛程度。结果在时间为2h、4h、8h、16h时,4L/min组较2L/min组疼痛程度下降,差异有统计学意义(P<0.05);6L/min组和8L/min组较2L/min组疼痛程度下降,差异有统计学意义(P<0.05),与4L/min组比较差异无统计学意义(P>0.05)。在32h时,四组间颈肩部疼痛程度比较差异无统计学意义(P>0.05)。结论中流量吸氧能降低腹腔镜肾癌根治术后颈肩部疼痛的发生率,改善腹腔镜肾癌根治术后颈肩部疼痛程度。Objective To compare the effect of different inhaled oxygen concentration on relieving pain in neck and shoulder after radical nephrectomy for carbon dioxid pneumoperitoneum. Methods 60 patients receiving radical nephrectomy were divided into the 2L/min group ( Group A, n = 15 ), the 4L/min group ( Group B, n = 15 ), the 6L/min group( Group C, n = 15 ) and the 8L/min group( Group D, n = 15), and all of them received continuous oxygen inhalation 32 hours after radical nephrectomy. The incidence and degree of pain in neck and shoulder after 2h, 4h,8h and 16h of radical nephrectomy were analyzed. Results When measured after 2h,4h,8h and 16h of radical nephrectomy , the degree of pain in neck and shoulder in Group B was significantly lower than that in Group A( P 〈 0.05). Group C and Group D both had significantly lower degree of pain in neck and shoulder than Group A ( P 〈 0.05 ), but had no significant difference with Group B( P 〉 0.05 ). After 32h ,the four groups showed no significant difference ( P 〉 0.05 ). Conclusion Medium flow of oxygen inhalation can reduce the incidence and degree of pain in neck and shoulder after radical nephrectomy for carbon dioxid pneumoperitoneum.
