超声生物显微镜引导下折叠人工晶状体睫状沟缝线固定术临床观察

摘  要:目的通过对比性临床观察,评价超声生物显微镜(UBM)引导下定位睫状沟,行折叠人工晶状体睫状沟缝线固定术的手术效果,为临床应用提供依据。方法临床病例对照研究。对2010年4月至2012年1月在郑州大学第一附属医院眼科将34例(37只眼)需行人工晶状体睫状沟缝线固定术的患者按术前应用UBM检查和未用UBM检查分为两组:I组18例19只眼,术前行UBM检查,定位睫状沟在巩膜表面投影位置,依此确定手术方位。Ⅱ组16例18只眼术前未行UBM检查按传统位置行人工晶状体睫状沟缝线固定术。两组所有患者术后3月行UBM及对比敏感度检查,观察患者手术进针位置、人工晶状体襻的位置、人工晶状体倾斜度、偏心量、视力、对比敏感度,并对相关数据进行统计学分析。结果两组之间在手术进针位置、术后3月人工晶状体偏心量及倾斜度、人工晶状体襻在睫状沟内的比率、裸眼视力差异均有统计学意义(P〈0.05),术后3月最佳矫正视力及各空间频率对比敏感度差异无统计学意义(P〉0.05)。结论UBM引导下行折叠式人工晶状体睫状沟缝线固定术,能够提高人工晶状体襻植入睫状沟的准确度,减少术后人工晶状体偏心量、倾斜度。Objective To evaluate the clinical effectiveness of foldable intraocular lenses in cili- ary sulcus suture fixation with ultrasound biomicroscope-guided, for clinical application. Methods This study was performed on 37 eyes in 34 patients who needed a foldable intraocular lens in cili- ary sulcus suture fixation surgery. A total of 19 eyes in 18 patients had a ultrasound biomicroscope (UBM) examination to ensure the ciliary sulcus localization before the surgery (Group | ). A total of 18 eyes in 16 patients didn't have a UBM examination (Group ]] ). After the surgery, the visual acuity, contrast sensitivity, UBM were checked up in both groups. The results were analyzed by SPSS17.0 statistical software. Results The mean suture position in group I was significantly less than that of the group ]I (P 〈0.001, t-test). The UBM examination showed that the IOL decentra- tion, the optic tilt and the rate of haptics located in the sulcus were statistically significant between the 2 groups. The uncorrected visual acuity (UCVA) 3 months after operation had statistical signifi- cance between the 2 groups. But the best corrected visual acuity (BCVA) and the contrast sensitivity function in 3cpd, 6cpd, 12cpd and 18cpd spatial frequencies 3 months after operation had no signifi- cant difference between the 2 groups. Conclusions The use of UBM can help the surgeon identifythe ciliary sulcus more precisely, and reduce the tilt and decentration of the IOL, which is condu- cive to upgrade the operated effect.
