摘　　要：目的探讨肝动脉化疗栓塞术(TACE)联合氩氦冷冻治疗原发性肝癌的效果及患者细胞免疫功能变化。方法选择42例原发性肝癌患者随机分为2组:A组共22例,行TACE联合氩氦冷冻消融术;B组共20例,单纯行TACE治疗。观察2组治疗有效率及血清AFP水平变化,同时分别检测患者治疗前、治疗后2周T细胞亚群CD3+、CD4+、CD8+的变化;同期选择正常健康体检者30名为对照组。结果 TACE联合氩氦冷冻治疗的有效率明显高于单纯TACE治疗组,TACE联合氩氦冷冻治疗患者AFP水平显著低于单纯行TACE组患者;原发性肝癌患者治疗前CD3+、CD4+、CD4+/CD8+T细胞值均低于正常对照组(P均<0.05),CD8+T细胞值高于正常对照组(P<0.05);TACE联合氩氦冷冻治疗后患者CD3+、CD4+、CD4+/CD8+T细胞值显著增加(P均<0.05),CD8+T细胞显著下降(P均<0.05);单纯TACE治疗后患者CD3+、CD4+、CD4+/CD8+T及CD8+T细胞值变化均不显著(P均<0.05)。结论 TACE联合氩氦冷冻治疗原发性肝癌的疗效优于单纯TACE治疗,TACE联合氩氦冷冻治疗能够提高原发性肝癌患者的细胞免疫功能。Objective To investigate the efficacy and variations of cellular immunity function in patients with primary hepatic carcinoma treated by TACE combined with argon-helium cryoablation. Methods The 42 primary hepatic carcinoma patients were randomly divided into two groups： group A （n=22）, treated by TACE combined with argon-helium cryoablation and group B（n=20）, treated with alone TACE. The efficacy and the level of AFP in serum of patients were observed. The level of T-cell subsets of patients, CD3, CIM and CD8, was detected by flow cytometry before treatment and 2 weeks healthy people were evaluated as control. Results The after treatment, respectively. The immunity function of 30 respond rate of group A was higher than group B, and the level of AFP of patients decreased in group A than that of in group B notably. CD3~, CD4~ and CIM~/CD8~ T cell in primary hepatic carcinoma patients were lower than control group （P〈O.05） before treatment, and CD8~ T cell were higher. After treatment, CD3＋, CIM＋ and CD4＋/CD8＋ T cell were increased （P〈0.05）, and CD8~ T cell decreased significantly（P〈0.05） in patients of group A. There were no changes in group B（P〉0.05）. Conclusion The treatment of TACE combined with argon-helium cryoablation has more efficacy than TACE alone. TACE combined cryoablation treatment enhances the cellular immunity function of primary hepatic carcinoma patients.
