某县乡村医生基本情况、健康服务工作量和满意度调查

摘　　要：目的了解乡村医生(以下简称村医)的基本情况、健康服务工作量和自身满意度。方法采用课堂集中问卷填写的方式对某县1046名村医进行调查。结果基本情况调查表明,村医中男性占65.3%,40～60岁年龄组人数占42.3%,46.9%的村医从医年限在20～50年,70.1%的村医具有中专文化,69.8%的村医毕业于卫校。健康服务工作量调查显示,村医平均每天的工作时间在8小时以上者达86.3%,85.1%的村医平均每天的休息时间小于8小时,40.4%的村医平均每天诊治病人可达到10～20名,平均处理每个病人的时间在5～10分钟者占47.9%。自身满意度调查分析发现,96.1%的村医认为工作有意义,79.1%的村医喜欢村医工作,64.0%的村医没有调换职业的打算。面对一天的工作有65.1%的村医感觉到累,93.1%的村医能认真对待病人,90.1%的村医担心工作中出现差错。面对不合作的病人,47.8%的村医感觉没有耐心且有51.2%的村医对病人曾产生埋怨。然而,有76.5%的村医对自己目前的工作环境感到满意,90.0%的村医与同事协作愉快,87.2%的村医认为医患关系和谐。但是,不满意自己的收入者占86.8%,90.7%的村医担心以后养老没有保障。结论该县男女村医比例合适,年龄结构偏大,文化程度较低。大多数村医满意目前的工作、环境及同事关系和医患关系,都能尽职尽责,积极做好基础医疗卫生服务工作,但村医工作量大,休息时间少,稳定工资收入较低,养老没有保障是突出问题。建议加强村级卫生服务网络和能力建设,改善村医生活和工作条件,建立健全村医的收入和保障体系,提高基层卫生服务水平。Abstract OBJECTIVE To understand the status and health service workload and satisfaction of village nurses. METHOD In classroom concentration questionnaires way, to carry out an investigation on the basic conditions and health services workload and their own satisfaction of 1 046 village doctors. RESULTS The basic situation survey showed that 65.3% village nurses are male, 40-60 age group accounted for 42.3%; 46.9% village nurses＇ working life are 20-50 years; 70.1% village nurses has secondary education,69.8% village nurses graduated from secondary vocational schools.Health service workload survey showed that 86.3% the village nurses＇ average daily working time is more than 8 hours,85.1% village nurses average daily rest time is less than 8 hours. 40.4% village nurses treat 10-20 patients every day,the average processing time for each patient in 5-10 minutes accounted for 47.9%.Own satisfaction survey showed that 96.1% village nurses believe that their work is meaningful, 79.1% village nurses like their job, 64.0% village nurses was not intended to change their work. 65.1% village nurses feel tired after a day＇s work. 93.1% village nurses take the patient seriously.Faced with uncooperative patients, 47.8% village nurses feel no patience, and 51.2% village nurses had complained.However, 76.5% village nurses are satisfied with their current work environment, 90.0% village nurses collaborate with colleagues happily, 87.2% village nurses think that the doctor-patient relationship is harmony. Yet, 86.8% village nurses were not satisfied with their income and 90.7% of village nurses worried that their pension was not guaranteed. CONCLUSION An appropriate proportion of men and women, larger age structure, lower educational level of the village nurses. Most village nurses satisfied with their work environment, the colleagues＇ and doctor-patient relationship, carried actively basic medical and health services, but the heavy workload , less rest time, low wages, no guaranteed pension were outstanding issues.
