原发性肝癌患者髓过氧化物酶活性和肿瘤坏死因子-α水平变化及临床意义

摘　　要：目的探讨原发性肝癌(PHC)患者髓过氧化物酶(MPO)活性和肿瘤坏死因子-α(TNF-α)水平的变化及临床意义。方法选择51例PHC患者手术切除的癌变组织及距离肿瘤边缘5 cm以上的正常良性肝组织,采用比色法检测组织中MPO活性,免疫组织化学法检测TNF-α的表达。结果肝癌组织中MPO活性和TNF-α的表达显著低于癌旁良性肝组织(P<0.01);高、中分化肝癌组织中MPO活性显著高于低分化肝癌组织(P<0.01),有远处转移的肝癌组织中MPO活性显著低于无远处转移肝癌组织(P<0.05)。TNF-α在高、中分化肝癌组织中的阳性表达显著高于低分化肝癌组织(P<0.01),有远处转移的肝癌组织中TNF-α阳性表达水平显著低于无远处转移的肝癌组织(P<0.01)。结论 MPO活性和TNF-α的表达可以作为判断PHC发生发展、分化程度及转移的检测指标。Objective To investigate the changes and clinical significance of myeloperoxidase（MPO）activity and tumor necrosis factor-α（TNF-α）level in patients with primary hepatic cancer（PHC）.Methods Fifty-one samples of cancerous tissues and normal benign hepatic tissues（more than 5 cm from the edge of the tumor）were selected respectively.The activity of MPO was detected by chromatometry and the level of TNF-α was detected by immunohistochemical method.Results The MPO activity and TNF-α level in hepatic cancer tissues were significantly lower than those in adjacent normal hepatic tissues（P〈0.01）,the MPO activity in well and moderately differentiated hepatic cancer tissues was significantly higher than that in poorly differentiated hepatic cancer tissues（P〈0.01）,and the MPO activity in hepatic cancer tissues of patients with distant metastasis was significantly lower than that in hepatic cancer tissues of patients without distant metastasis（P〈0.05）.The positive expressions of TNF-α in well and moderately differentiated hepatic cancer tissues were significantly higher than that in poorly differentiated hepatic cancer tissues（P〈0.01）,and the positive expression of TNF-α in hepatic cancer tissues of patients with distant metastasis was significantly lower than that in hepatic cancer tissues of patients without distant metastasis（P〈0.01）.Conclusion MPO activity and TNF-α level can be used as the indicators for evaluating the development,differentiation and metastasis of PHC.
