摘  要:目的:探讨褪黑素对大鼠急性脊髓损伤后白细胞介素-10(IL-10)表达的影响,以及对损伤后炎症反应、神经修复的作用。方法:108只成年SD大鼠随机分为模型组、治疗组和对照组3组。前2组用改良的WD法建立T12脊髓损伤模型,对照组仅切除T12椎板不损伤脊髓。治疗组在脊髓损伤后10min注射褪黑素(100mg/kg),模型组注射等量的体积分数5%乙醇。术后第1、2、3、5、7天对3组大鼠进行神经功能评分,检测血清中IL-10含量和脊髓中IL-10mRNA的水平。3d时各组选6只行主动脉灌流,后取脊髓进行HE染色,免疫组化SP法染色观察IL-10的表达特点。结果:模型组和治疗组大鼠术后均出现了严重的下肢神经功能障碍,BBB运动功能评分早期差异无统计学意义(P>0.05),术后3天、5天、7天两组运动功能评分差异有统计学意义(P<0.01)。术后同时间点治疗组脊髓内IL-10mRNA水平均高于模型组和对照组,这与血清中IL-10含量及表达IL-10阳性细胞数结果一致,均有治疗组最高、模型组次之、对照组最低,各组之间差异具有统计学意义(P<0.01);免疫组化见IL-10在中央管室管膜细胞、神经细胞、神经胶质细胞胞浆中分布较多。结论:褪黑素可能通过提高中央管周围内皮细胞、神经细胞、胶质细胞对IL-10的分泌来抑制脂质过氧化反应和炎症反应。Aim: To explore the effects of melatonin ( MT) on expression of interleukin-10( IL-10) in acute spinal cord injury ( SCI) in rats,and to investigate the function of MT in the inflammatory response and nerve repair after acute SCI. Methods: A total of 108 Sprague-Dawley ( SD) rats were randomly divided into 3 groups: model group,treatment group and control group. The first two groups acute SCI model were established by using the modified Allen's weight drop ( WD) method at T 12 level,and the control group was removed lamina only. At 10 min after the model establishment,5% ethanol was given to the model group and 100 mg / kg of MT to treatment group. At 1 d,2 d,3 d,5 d,7 d,the function of the spi-nal cord was assessed by Basso Beattie Bresnahan locomotor rating scale ( BBB scale) ,the blood samples and spinal cord were obtained to measure the levels of IL-10 and IL-10 mRNA. At 3 d,6 samples of spinal cord from aorta perfusion model were taken in each group for histological examination using HE and Immunohistochemical SP staining methods. Results: Rats of model group and treatment group showed severe lower limb neurological dysfunction after surgery. At 3 d,5 d and 7d,the difference between them in BBB scale had statistical difference ( P 0. 01) . IL-10 mRNA levels of treatment group was highest at each time point,and it was consistent with the IL-10 levels at serum and the IL-10 positive cells. The differ-ence between each two groups had statistical significant ( P 0. 01) ,immunohistochemistry showed that more of IL-10 dis-tributed at the cytoplasm of endothelial cell,nerve cells and glial cell. Conclusion: This study indicates that MT suppress lipid oxidation reactions and inflammatory response by increasing the secretion of IL-10 in central canal surrounding endo-thelial cell,nerve cells and glial cell.
