摘  要:目的研究肝细胞癌干细胞标记分子CD133与血管内皮生长因子VEGF在肝细胞癌组织中的表达及其预后价值。方法应用免疫组织化学染色方法检测CD133、VEGF在190例肝细胞癌组织中的表达水平,分析其与各项临床病理指标和无瘤生存之间的关系。结果 CD133、VEGF在肝细胞癌组织中的阳性表达率分别为22.1%(42/190)和52.1%(99/190)。CD133的表达水平与乙型肝炎病毒感染、肝细胞癌分化程度、镜下血管侵犯比较,差异有显著统计学意义(P<0.05);CD133表达与VEGF表达水平比较,差异有显著统计学意义(P<0.001)。生存分析表明,CD133阳性组术后无瘤生存期明显短于CD133阴性组,差异有显著统计学意义(P=0.036)。结论肝细胞癌组织中,CD133高表达与VEGF高表达、肝细胞癌低分化以及镜下血管侵犯密切相关;CD133表达水平与肝细胞癌术后无瘤生存呈负相关,CD133可能通过调节VEGF表达水平影响肿瘤局部新生血管形成从而影响肝细胞癌预后。Objective To investigate the expression of CD 133 and vascular endothelial growth factor (VEGF) in primary lesions of hepatocellular carcinoma (HCC) and its prognostic value. Methods The expression of CD133 and VEGF in 190 patients with HCC were detected by immunohistochemical staining. The correlationship of CD133 expression with the clinicopathologic parameters and features after operation was analyzed. Results The expression of CD133 and VEGF in cancer tissues was 22.1% (42/190) and 52.1% (99/190), respectively. There was significant correlationship between the expression of CD 133 and HBV infection, tumor differentiation and microvessel invasion (P 〈 0.05). There was significant correlationship between the expression of CD 133 and VEGF (P 〈 0.001). Prognostic analysis demonstrated that the disease-free survival of CD133 positive group was significantly lower than that of CD133 negative group (P = 0.036). Conclusions CD133 expression in HCC correlates significantly with VEGF expression, worse tumor differentiation and microvessel invasion. Patients with CD133 positive expression may suffer from shorter disease-free survival after operation. CD133 may influence the prognosis of HCC by regulating the expression of VEGF.
