45例额叶癫痫患者的临床特征及脑电图分析

摘　　要：目的探讨额叶癫痫患者发作时的临床特征与脑电图改变。方法对2010年5月—2012年1月在该院治疗的45例额叶癫痫发作患者临床特征及脑电图改变进行分析。结果 45例癫痫患者共监测到107次临床发作,平均2.4次/例,其中睡眠期间发作78次(72.9%),清醒期发作29次(27.1%),临床表现的发作类型主要有:过度自动运动症、全身强直阵挛、口咽自动症、偏转性强直、发声发作、失张力发作等,其中发作次数最多的前3位分别是发声发作、偏转性强直和全身强直阵挛;额叶癫的脑电图特征为放电部位以额区为主,占到80.00%,发作频率主要是偶发/阵发,占88.89%,节律主要以阵发性棘(尖)波或棘(尖)慢波为主占62.22%。结论额叶癫痫临床表现具有一定的共同特征:发声发作、偏转性强直和全身强直阵挛较常见,多在夜间发作,脑电图以额区偶发/阵发性棘(尖)波或棘(尖)慢波为主。Objective To explore the clinical features and EEG for patients with frontal lobe epilepsy. Methods The clinical features and EEG for 45 cases of patients with frontal lobe epilepsy from May 2010 to January 2012 were analyzed. Results There were a total of 107 clinical onset monitored in the 45 cases, an average of 2.4 times/cases, including the onset of sleep was in 78 cases （72.9%）, a clear onset was of 29 cases （27. 1%）. The main clinical manifestations of the onset of type were： excessive auto- movement disorder, generalized tonic-clonic, oropharyngeal automatically disease, the deflection ankylosis voice seizures, atonic seizures, etc, number of episodes up to the top three were vocal attack, deflection tonic and generalized tonic-clonic. The frontal mainly was frontal epileptic EEG discharge site, accounting for 80.00%. The seizure frequency was sporadic / burst, accounting for 88.89%. The rhythm, slow paroxysmal spike （sharp） waves or spines （tip） wave were main, accounted for 62.22%. Conclusion The clinical manifestations of frontal lobe epilepsy have certain common features： voice attack, deflection tonic and generalized tonic- clonic are more common. It attacks mainly in night. The EEG are mainly based of frontal episodic / paroxysmal spike-wave （sharp） or spine （tip） slow-wave.
