儿童自我报告结局测量信息系统的已知组别效度分析

摘　　要：目的检验儿童自我报告结局测量信息系统八大简表的可行性及已知组别效度。方法 2012年5-9月,采用便利抽样法选取上海市和成都市4所儿童医院8～17周岁的232名儿童和青少年癌症患者为研究对象,应用一般资料调查表、儿童自我报告结局测量信息系统八大简表对其进行调查,通过问卷有效回收率及回答有效性来分析其可行性,采用t/U检验来检验治疗期组和无进展生存期组的组间差异。结果本次调查共发放问卷247份,回收有效问卷232份,有效回收率为93.93%。治疗期组与无进展生存期组儿童的抑郁症状、愤怒、疲劳、疼痛影响、同伴关系、身体功能-移动性、身体功能-上肢功能七大简表测评结果的差异有统计学意义(P<0.05或P<0.01),儿童焦虑简表的测评结果差异无统计学意义(P>0.05)。结论儿童自我报告结局测量信息系统简表具有良好的可行性,大部分简表具有良好的已知组别效度,可进行该系统简表的其他测评,进而为儿童癌症患者提供可比、精确的健康相关测评工具。Objective To examine the feasibilities and known-group validities of eight short forms of Chinese version of pediatric patient-reported outcomes measurement information system（PROMIS）. Methods From May to September 2012, 232 cancer patients aged 8-17 in 4 children hospitals of Shanghai and Chengdu were investigated with general information questionnaire and eight short forms of pediatric PROMIS. The feasibilities of the results were analyzed based on the valid response rate to the questionnaire and the validity of answers. T/U test was used to test the difference between the treatment group and disease-free survival group. Results Of 247 questionnaires distributed in the survey, 232 valid questionnaires were collected and the effective response rate was 93.93%. The distinction of seven short forms of pediatric depressive symptoms, anger, fatigue, pain impact, peer relationship, physical function-mobility and physical function-upper extremity were significant between the treatment group and disease-free survival group（P〈0.05 or P〈0.01）. However,no significant difference was detected on the results of pediatric anxiety short forms（P〉0.05）. Conclusion The results indicate good feasibility of pediatric PRO- MIS,and good known-group validities of most short forms. Other measurement examination are suggested to provide comparable and accurate instruments for cancer children.
