重症急性胰腺炎早期肠内免疫营养治疗的价值

摘　　要：目的探讨早期肠内免疫营养治疗在重症急性胰腺炎(SevereAcute Pancreatitis,SAP)治疗中的重要价值。方法对2007年9月至2012年8月昆明市第一人民医院收治的SAP进行回顾性分析,将106例患者随机分为对照组(41例)和免疫营养组(65例),在发病后2～3d置入鼻空肠管管饲,观察体质量,血红蛋白,白蛋白,总淋巴细胞计数及后续各种并发症发生情况。结果对照组治疗前后白蛋白无差异,P>0.05,治疗后总淋巴细胞计数P<0.05,治疗组在治疗前后白蛋白,总淋巴细胞计数,血红蛋白,P<0.01,有显著性差异。对照组死亡6例,其主要死因是多脏器功能衰竭(MOF)。结论在SAP综合治疗中,早期进行免疫营养支持是切实可行的,对防治感染及多脏器功能不全(MODS)有重要的临床价值。Objective To discuss the significance of supplementing intraintestinal immunizational nutrition in the early stage of treating Severe Acute Pancreatitis（SAP）. Methods To divide randomly 106 cases of SAP into control group（41 cases） and immunizational nution group（65 cases）, who are hospitalized in our hospital from September, 2007 to August, 2012.2-3 days after the morbility, the therapy group is put in the nasojejunal tube to have gavage. The weight, hematoglobin, albumen, total number of lymphocyte and subsequent complicating diseases are observed. Results The control group has no differentiation of albumen before and after the therapy, P〉0.05. After the the therapy, the total number of lymphocyte, P〈0.05.As to the therapy group, the albumen, total number of lymphocyte, hematoglobin show significant differentiation before and after the therapy, P〈0.01. There are 6 cases of death in the control group,who primarily died of polyorganichpofunction. Conclusion During the comoprehensive treatment of SAP, the early Supplementing of immunizational nutrition is very important, which is workable It also has significant clinical value in preventing and treating infection and MODS.
