腹部肿瘤手术后应激性溃疡的预防和治疗

摘　　要：目的　通过对 166例腹部手术后病人的治疗进行分析和总结 ,以探讨术后应激性溃疡的预防和治疗。方法　 5 9例术后发生应激性溃疡的病人 ,在输血补液抗休克治疗、胃肠减压、胃内局部用药等措施的基础上 ,分别应用H2受体阻断剂 -甲氰咪胍和洛赛克进行治疗 ,分析治疗结果。另外有 10 7例腹部大手术后及发生较严重并发症的病人术后短时间应用洛赛克预防应激性溃疡的发生。结果　 1992年以前的 3 3例应激性溃疡病人应用甲氰咪胍治疗 ,治愈 2 5例 ,死亡 8例 ,治愈率为 75 .8%。 1992年后 2 6例应激性溃疡病人应用洛赛克治疗 ,全部治愈。另外应用洛赛克预防治疗的 10 7例病人于出院前均未出现应激性溃疡。结论　应激性溃疡是外科术后常见并发症 ,如预防得当、诊断及时、治疗合理 ,可以得到满意效果。洛赛克对腹部外科术后应激性溃疡的治疗及预防效果均佳。? Objective To probe the prevention and treatment of postoperative stress ulcer in abdominal surgery.Methods 59 cases were divided into 2 groups,treated by H 2 receptor antagonist cemitidine or losec (omeprozole)respectively.Furthermore,there were 107 patients treated by losec to prevent stress lucer.Results 33 cases before 1992 were treated by cemitidine,among whom 25 cases were cured and 8 cases died.Effective rate was 75.8%.26 cases treated by losec after 1992 all were cured.Meanwhile,the 107 patients prevented by Losec didnt suffered from the complication.Conclusions Stress ulcer is a common postoperative complication in abdominal surgery.The effect of treatment was influenced by prophylactic,diagnostic and therapeutic measures.Losec is a good measure to treat or prevent postoperative stress ulcer in abdominal surgery. 〔
