舌诊在慢性萎缩性胃炎辨证中的意义

摘　　要：目的:探讨舌诊在慢性萎缩性胃炎(CAG)辨证中的意义。方法:以102例CAG病例的临床数据为基础,观察舌象特征及胃黏膜幽门螺旋菌(Hp),分析CAG各证型舌象差异。结果:CAG不同证型的舌象变化差异有统计学意义(P<0.01)。舌质:脾胃虚弱型以淡白舌多见,脾胃湿热型以红舌为主,胃阴不足型出现红舌或红绛舌,肝胃不和型可见淡红舌,胃络瘀血型见紫(暗)舌。舌苔:薄白苔多见于早期或恢复期;舌苔黄腻或变为灰黑苔,多提示病情严(加)重。Hp感染:阳性率从高到低对应的舌苔依次是灰黑苔>黄厚腻>白厚腻>薄黄>薄白。结论:在CAG辨证中,舌诊有助于区别病因病机、辨别病位深浅、推断病势进退以及估计病情预后。Objective： To explore the significance of tongue diagnosis in chronic atrophic gastritis （CAG） dialectical. Methods： The clinical data of 102 cases of patients with CAG observed characteristics of the Tongue and gastric Helicobacter pylori （Hp）, analysis of CAG the various syndromes Tongue difference. Results： The CAG of different syndromes Tongue change significant difference （P 〈 0.01 ）： Tongue quality： Syndrome of weakness of the spleen and stomach mainly with pale tongue; syndrome of splenogastric hygropyrexia mainly with red tongue; syndrome of yin deficiency of stomach mainly with red tongue and deep red tongue; syndrome of incoordination between the liver and stomach mainly with pale tongue and pink tongue; syndrome of blood stasis in stomach collaterals mainly with purple （dark） tongue. Tongue coating： thin and whitish fur was mainly observed in early stage or convalescence stage; persisted yellow-greasy tongue coating or yellow-greasy tongue coating changing into gray-black fur was mainly observed in severe period. Helicobacter pylori test： positive rate form high to low were gray-black fur, yellow-thick fur, white-thick fur, thin- yellow fur and thin-white fur. Conclusion： The CAG Syndrome, tongue inspection can distinguish nature of pathogenic factor, identify disease position, infer tendency of disease and estimate prognosis.
