超声乳化术后眼内炎6例临床分析

摘　　要：目的探讨白内障超声乳化术后眼内炎的早期诊断及治疗。方法回顾性分析2010年10月至2012年4月在我院就诊的6例(6只眼)白内障超声乳化术后眼内炎患者的临床表现、治疗及预后。结果 6例患者中1例采用全身抗菌治疗及局部频点抗菌滴眼液保守治疗,其余5例在保守治疗的基础上全部采用前房灌洗及头孢唑啉联合万古霉素玻璃体腔注射,其中3例单纯采用玻璃体腔注药,次数为2～4次,余2例采用玻璃体注药联合玻璃体切除术。视力恢复除1例婴幼儿无法检查外,其余均有不同程度的提高。无1例摘眼。结论超声乳化术后眼内炎的早期诊断及治疗尤为重要;头孢唑啉联合万古霉素玻璃体腔注射及必要时玻璃体切除术是治疗超声乳化术后眼内炎的有效方法。Objective To discuss the early diagnosis and treatment of endophthalmitls after phacoemulsification. Methods Retrospective analysis of the clinical featmes, physical signs, treatments and profnosis of 6 cases of endoph- thalmltis after phacoemulsification from October, 2010 to April, 2012. Results One of the 6 cases was euoxi by whole- body antibiotics and local fiequeut eyedrops treatment. Based on conserva6ve treatment, the others were all treated by ante- riot chamber irrigation and intravitreal injection of cefazolin and vancomycin, among which 3 cases were only given intravit- real injection, 2 to 4 times; another 2 cases were given intravitreal injection combined with vitrectomy. Visual acuities d all cases were improved except one infant case that was too young to be examined. No one was plucked eye. Conclusion The early diagnosis and treatment of endophthnlmitis after phacoemulsification are very important, intravitreal injection of cefazolin and vancomycln and timely vitrectomy are the effective methods.
