摘 要:目的研究尿中可溶性Fa(ssFas)在膀胱癌诊断中的价值。方法采用临床流行病学病例对照研究方法,随机抽取承德医学院附属医院泌尿外科2008年1月~2011年5月住院的膀胱癌病人和对照各68例,采用ELISA法测定标本尿中sFas水平,根据性别、年龄、膀胱癌病理分级和分期进行分析。根据ROC曲线中敏感性和特异性之和最高点为参考,制定尿中sFas的界值并计算ROC曲线下面积,分析诊断效率。结果尿sFas在膀胱癌病例组为(148.0±64.4)ng/mL,对照组为(58.5±35.0)ng/mL,差异有显著性(P〈0.01)。尿sFas随着病理分期、分级的增加而增加(P〈0.05),与年龄、性别无关(P〉0.05)。根据制定的ROC曲线,尿sFaS在本次实验中的界值为91.0 ng/mL,曲线下面积为0.928(P〈0.01),灵敏度为85.3%,特异度88.2%(P〈0.01)。结论尿液sFas检测技术具有敏感性高、无创伤、可定量分析等优点,可能作为诊断膀胱癌的一个指标。[ Objective ] To evaluate the clinical use of urine soluble Fas (sFas) for diagnosis of bladder transi- tional cell cancer (BTCC). [ Methods ] The case-control study of clinical epidemiology was carried out. There were 68 cases with BTCC and control group from department of urology in affiliated hospital of Chengde Medical College at random. Then the level of sFas was measured in the urine by enzyme-linked immunosorbent assay (ELISA). sFas was analyzed according to gender, age, pathological grading and staging. The cut-off values and area under curve (AUC) of sFas was obtained by receiver operating characteristic (ROC) curve. The diagnostic efficiency of the tumor marker for bladder cancer was assessed by the fourfold table. [Results] The value of urine soluble Fas was (148.0_+ 64.4) ng/mL in the BTCC cases and (58.5~35.0) ng/mL in control groups, respectively (P 〈0.01). The level of sFAS was elevated with the increase of pathological stage and grade. The value of sFAS in invasive BTCC cases was sig- nificantly higher than that in non-invasive cases (each P 〈0.01), and the value was not significantly different at dif- ferent gender, ages (each P 〉0.05). Based on the ROC curve, the cut-off values and AUC of sFAS were 91.0 ng/mL and 0.928 in this test, respectively (P 〈0.01). According to the standard, the sensitivity and specificity of sFAS for diagnosis of bladder cancer was 85.3% and 88.2%, respectively (P 〈0.01). [Conclusions] Urinary soluble Fas is a simple, noninvasive, sensitive test for the diagnosis of BTCC.
