摘 要:目的探讨经PICC导管化疗对肿瘤患者血液流变学及输液相关并发症的影响。方法将2011年2月~2012年2月住院的194例肿瘤化疗患者分为2组:对照组98例,经外周静脉留置针进行化疗;实验组96例,经PICC导管进行化疗。分别于化疗前和化疗4个疗程后,测量并比较两组患者的各项血液流变学指标和局部皮肤、血管等并发症状况。结果经协方差分析,化疗后实验组患者仅在全血高切相对指数一项指标上高于对照组患者,二者差异有显著性(P=0.038),其他血流变指标均差异无显著性(P〉0.05)。经PICC化疗后实验组患者在臂围增粗、皮温升高、皮色变化、穿刺点红肿、自觉不适症状与疼痛的发生率均低于对照组患者,差异具有显著性(P〈0.01),而穿刺点感染发生率两组差异没有显著性(P=0.495)。结论经PICC导管化疗基本没有使患者的血液流变学指标发生明显异常,并且能够有效降低输液相关并发症发生率。[Objective] To explore the influence of chemotherapy intravenous infusion by placement of peripheral- ly inserted central catheter on the hemarheology and related complications of cancer patients. [ Methods ] Totally 194 cancer inpatients were divided into two groups. The 98 patients in control group were applied to traditional in- travenous catheter. The 96 patients in experimental group were applied to peripherally insert central catheter. The hemodynamic indexes and the situation of local skin and blood vessel as well as other complications were compared between the two groups. [ Results ] The mean of the whole blood relative viscosity of the patients in the experimental groups was a little higher than that of the control groups. The difference of this hemodynamic index between the two groups had obvious statistics significance (P =0.038). The means of the other 13 hemodynamic indexes between the two groups had no statistics significance (P 〉0.05). The differences of arm circumference, skin temperature, and skin color, the situation of blood vessel, subjective symptom and the sense of pain between the two groups had obvious statistics significance (P 〈0.01). The incidence of complications mentioned above in experimental groups was much lower than control groups. While the incidence of local infection between the two groups had no statistics signifi- cance (P =0.495). [Conclusion] Peripherally inserted central catheter could effectively protect the local skin and blood vessel, decrease the incidence of complications related to chemotherapy intravenous infusion and relieve the sense of pain dramatically, while it basically didn't change the hemodynamic indexes of the patients distinctly com-pared with the traditional intravenous catheter.
