术前使用卡培他滨联合顺铂新辅助化疗治疗宫颈癌的临床疗效研究

摘　　要：目的评价根治术前使用卡培他滨联合顺铂新辅助化疗治疗宫颈癌的近期临床疗效。方法选择根治术前Ⅰb2~Ⅱa期宫颈癌患者60例,按治疗方法不同分为两组：接受顺铂＋卡培他滨2~3个疗程后行根治性手术30例（研究组）,同期未化疗直接手术者30例（对照组）,对研究组的化疗有效率、毒副反应进行统计,对两组根治术时间、术中出血量、并发症、术后病检结果进行比较分析。结果术前顺铂联合卡培他滨新辅助化疗总有效率（CR＋PR）78.1%,明显提高了单药的效率;化疗后毒副反应只有I、II级,无III、Ⅳ级。术中出血情况及术中时间并无明显差别,术后病理结果发现,卡培他滨联合顺铂新辅助化疗的治疗效果较佳。结论卡培他滨联合顺铂新辅助化疗方案明显使肿瘤病灶缩小,化疗毒副反应小,对于治疗宫颈癌是有效、安全的。[ Objective ] To evaluate the recently clinical efficacy of capecitabine combined with cisplatin neoad- juvant chemotherapy in cervical cancer before surgery. [Methods] 60 patients with I b2-Ⅱ a cervical cancer before radical surgery were selected and divided into two groups according to the treatment methods： 30 cases received cis- platin ＋ capecitabine 2-3 courses then underwent radical surgery （study group）, 30 cases, over the same period, were directly to surgery （control group）. Statistical analysis was conducted on chemotherapy efficiency and toxicity, radical surgery time, blood loss, complications, and postoperative pathologic examination results of the two groups were comparatively analyzed. [Results] The total effective rate （CR＋PR） of capecitabine combined with cisplatin neoadjuvant chemotherapy was 78.1%. It significantly improved the efficiency of the monotherapy. And there was only I , ]I grade of side effects of chemotherapy, without m, IV grade. There was no significant difference between blood loss and intraoperative time. The pathologic results showed that the efficacy of the study group was more better. [ Conclusion ] Capecitabine combined with cisplatin neoadjuvant chemotherapy significantly reduced the tu- mor lesion with little side effect of chemotherapy. It is effective and safe for the treatment of cervical cancer.
