摘  要:目的探讨分化型甲状腺癌的规范化治疗方案。方法对2005~2010年我科收治的22例甲状腺结节术后病理诊断为分化型甲状腺癌的患者,行患侧甲状腺+峡部全切+对侧保留极少后被膜的近全切除+多功能保留性颈淋巴结清扫术,术后予以甲状腺素替代治疗,必要时加用131I去甲状腺治疗,同时监测三碘甲状腺原氨酸(T3)、四碘甲状腺氨酸(T4)、血清促甲状腺素(TSH)及血清甲状腺球蛋白(STG)的综合治疗和监测病情变化的临床资料进行回顾性分析。结果该组患者无手术死亡,手术后恢复较好,无神经损伤,颈部外观无塌陷。结论在治疗分化型甲状腺癌时,如果术式选择恰当加上合理的术后综合治疗与监测,既能达到治疗目的,又保留了颈部解剖结构和生理功能的完整性,提高患者术后生活质量,疗效满意。[ Objective ] To investigate the differentiated thyroid cancer with standard treatment plan. [ Methods ] During 2005 to 2010 in our department 22 cases of thyroid nodules pathologic diagnosis after operation for differenti- ated thyroid carcinoma were treated with ipsilateral thyroid isthmus resection++contralateral reserve some after mem- brane subtotal+muhi functional neck lymph node dissection, and postoperative to thyroxine replacement treatment, if necessary with iodine 131 to thyroid therapy, while monitoring the three triiodothyronine (T3 four), triiodothyronine (T4), serum thyroid-stimulating hormone (TSH) and serum thyroglobulin (STG) combination therapy and monitoring the condition changes were retrospectively reviewed of the clinical data. [ Results ] There were no operation death, after the recovery operation, no nerve damage, the exterior of the neck without collapse. [ Conclusion ] In the treat- ment of differentiated thyroid cancer, appropriate choice and reasonable postoperative comprehensive treatment and monitoring can achieve the purpose of therapy, hut also keep the cervical anatomic structure and physiological func- tion of integrity, improve patients quality of life after surgery with satisfying curative effect.
