房水引流物植入术后行其他眼手术发生的问题及处理

摘　　要：目的 :探讨房水引流物植入术后行其他眼科手术可能发生的问题及相应的处理方法。方法 :对我院 1994年 12月至 1999年 6月间进行各类青光眼房水引流物植入术 15 2例 ,术后需行其他手术的 10例患者 ,按常规手术和改良手术所得的结果进行分析评价。结果 :其中按常规手术的 3例患者 ,术中及术后均发生了严重的并发症 ,眼压升高达房水引流物植入前水平。其余 7例经过术中临时性结扎引流管、引流盘周引流区的保护等处理后 ,避免了术中及术后并发症的发生 ,术后追踪观察发现 5例患者眼压控制在 2 1mm Hg以下 ,占 71% ,2例患者加用局部降眼压药物后眼压控制在 2 1mm Hg以下 ,7例患者术后视力均较术前提高 ,获得了较好的手术效果。结论 :由于房水引流物植入术后改变了眼的正常解剖和生理特征 ,术后再行其他眼科手术时 ,必须对手术方式进行改进 ,改良后的手术方式对防止并发症的发生 。Object:To discuss the issues which occured during and after intraocular surgery in the patients who have been undertook glaucoma implant surgery so as to search an effective way to solve it.Methods:Retrospectively,to analyze intraocular surgery effect and complications of 9 cases from 152 cases who undertook glaucoma seton implant from Dec.1999 to June 1999 and the effectiveness of surgical manner.Classical and modified manners were compared.Results:Two cases with classical surgical manner have gotten serious complications in and after surgery.However,the other seven cases who undertook the modified operations have got better surgery effects and avoided the complications during and after the surgery.Conclusions:Because anatomical and physiological change of the eyes after glaucoma seton implantation,when these patients need to take other intraocular surgery again,the surgery method must be modified according to concrete needs.The surgical manner modified by the another is benefit to getting better surgery effects and preventing surgery complications.
