摘  要:目的探讨胸水LunX mRNA表达及DNA异倍体分析对非小细胞肺癌患者的临床指导意义。方法收集46例非小细胞肺癌患者胸水,22例非肺癌肿瘤胸水和15例肺部良性疾病胸水作为对照。采用荧光定量RT-PCR技术检测LunX mRNA表达情况,流式细胞技术检测胸水脱落细胞DNA倍体变化。结果良恶性胸水DI、增殖指数有显著差别((P<0.05),肺外肿瘤组和非小细胞肺癌组胸水DI、增殖指数无差别(P>0.05)。LunX mRNA阳性表达率在非小细胞肺癌组中为91.3%(42/46),显著高于其他两对照组(P<0.05)。25例NSCLC患者胸水LunX mRNA及DNA异倍体分析为共表达,显著高于其他两组(P<0.05)。结论通过对胸水LunX mRNA的检测联合DNA倍体分析可以提高肺癌患者胸水检测的阳性率和特异性,对非小细胞肺癌患者的诊断、复发、转移具有临床指导作用。Objective To evaluate the diagnostic values of DNA aneuploidy and Lunx mRNA expression of pleural fluid of patients with non small cell lung cancer (NSCLC). Methods Pleural fluid specimens were collected from 46 patients with NSCLC,22 pleural effusion samples of the tumor excluding lung tissue and 15 pleural effusion samples of benign disease were served as controls. Using fluorescence quantitative PCR technique for detection of LunX expression of mRNA,flow cytometry determination of pleural DNA ploidy analysis. Results There were significant differences in DNA index and proliferation index between pleural fluid of patients with NSCLC groups and benign disease(P〈0.05) ,the tumor excluding lung tissue have no difference with the NSCLC patients in DNA index and proliferation index . The positive rate of LUNX mRNA for NSCLC was 91.3% (42/46) and had a statistical significance with other two groups(P〈0.05). 25 in 46 patients with NSCLCdetected double positive results and showed a statistical Significance with other two groups(P〈0.05). Conclusion On the basis of pleural fluid LunX rnRNA detection and DNA ploidy analysis can improve the non small cell lung cancer patients with positive rate,for the judgment of non small cell lung cancer patients with recurrnce, metastasis and has important clinical significance. Joint detection of DNA-aneuploidy and LunX mRNA could improve the sensitivity and specificityof pleural fluid. They might be of high diagnostic values in NSCLC patients and might benefit further clinic procedures.
