DSA引导射频热凝术治疗三叉神经痛的临床观察

摘　　要：目的探讨在数字减影血管造影术(DSA)引导下定位经皮卵圆孔穿刺三叉神经半月节射频热凝术治疗三叉神经痛的临床疗效。方法对39例原发性三叉神经痛患者均采用DSA引导下Hartel前入路法穿刺准确到位后行半月节射频热凝治疗,观察患者症状缓解情况。结果 39例患者均准确穿刺到位,术后有效率97.4%。术后随访3～36个月,1例无效,1例复发,除了出现面部感觉障碍及角膜炎外,无其他严重并发症。结论在DSA引导下定位穿刺三叉神经半月节行射频热凝术治疗原发性三叉神经痛并发症少,安全可靠,疗效显著。Objective To evaluate the clinical effect of radiofrequency ablation under DSA guidance in treatment of primary tri-geminal neuralgia. Methods The modified Hartel method was used to puncture the trigeminal ganglion through. Thirty - nine patients received percutaneous radiofrequency ablation for the lesions branch of trigeminal and were observed the change of symptom. Results All patients were punctured successfully and postoperative efficient was 97.4%. The follow - up time was from 3 to 36 months. One pa-tient invalid and one patient recurred. No other major complications occurred despite of keratitis, facial numbness and weakness che- wing. Conclusion It is indicated that radiofrequency ablation under DSA guidance in treatment of primary trigeminal neuralgia is safe and effective treatment with low recurrence rate.
