腹膜后副节瘤的螺旋CT表现及诊断价值

摘　　要：目的研究腹膜后副节瘤的CT表现及图像后处理技术在外科手术中的价值。方法回顾分析8例经手术病理证实的腹膜后副节瘤的临床及CT表现。结果 8例中恶性1例,良性7例。病灶均为单发,呈圆形或椭圆形,最大径4～23 cm,平均10.6 cm。7例边界清晰,1例边界不清。3例肿瘤内见斑点状钙化,7例肿块见不同程度的出血、坏死和囊变,其中2例可见液-液平面,1例密度均匀。增强扫描肿瘤均富血供,强化程度40～110HU,以动脉期或静脉期为著,3例瘤体内或周围可见粗大血管影。4例血管重建清晰显示肿瘤的供血动脉、引流静脉及与周围相邻血管、器官的解剖关系。结论腹膜后副节瘤具有较特征性的CT表现,结合临床特征可在术前做出诊断。三维后处理技术可以很好地显示肿瘤血管及其与邻近结构的关系,为手术治疗提供重要依据。Objective To investigate CT findings of retroperitoneal paraganglioma and investigate value of post - procession technique in surgery. Methods Clinical presents and CT findings of pathological proven retroperitoneal paraganglioma in eight patients were retrospectively reviewed. Results Single tumor was detected in each of the eight patients. There were one malignant lesion and seven benign lesions. The tumors were round or oval with maximal diameter range of 4cm ~ 23cna and average diameter was I0.6 cm. Seven masses were well defined margin and one mass was ill - demarcated. Calcification was detected in three masses. There were hem- orrhage, necrosis and cystic changes in 7 tumors with liquid - liquid surface in two lesions, and one tumor was homogeneous density. All tumors showed avid enhancement after intravenous contrast materials. Enlarged vessels were seen within surrounding three tumors. Post procession could show supplying arteries and draining veins of the tumors clearly in 4 cases. Anatomy relationship between the tumors and adjacent organs and vascular was also well depictured. Conclusion Retroperitoneal paraganglioma has characteristic CT features. Correct diagnosis can be obtained by combining clinical presents and CT findings. Post - procession can depict anatomy relationship of tumor and surrounding organs and vessels clearly, which is helpful for surgery.
