不同年龄段行腭裂手术对颌骨发育及语音影响的临床研究

摘　　要：目的分析评价不同年龄段进行腭裂手术对颌骨发育及语音的影响。方法采用X线片头影测量方法对60例不同年龄段接受腭裂手术的患者颌骨特征进行比较分析。结果与正常组相比,腭裂患者上牙槽座角(SNA)、颌凸角(NA-PA)、上颌长(ANS-Ptm)、上颌位置(S-Ptm)和上面高(N-ANS)等结果偏小,差异有统计学意义(P<0.05);各实验组相互比较,腭裂患者在N-ANS及S-Ptm和N-ANS/N-Me3项上差异有统计学意义(P<0.05)。结论腭裂患者术后在上颌骨的长度、高度、宽度以及前后位置关系上存在不同程度的发育不足,但在下颌骨的高度和位置关系上则无明显差异。较小年龄段手术的患者在上颌骨的高度和前后位置关系上与较大年龄段手术的患者相比存在差异,但较小年龄组患者远期语音效果明显优于较大年龄组。Objective To analyze the influence on the jaw bone growth after cleft palate operation in different period. Methods By using X-ray cephalometry analysis, 60 patients with different ages of who accepted cleft palate surgery with mandibular characterized. Results Compared with the normal group, SNA, NA-PA, ANS-Ptm, S-Ptm and N-ANS of the cleft palate patients are smaller （ P 〈 0. 05 ）. The mutual comparisons in experimental groups showed S-Ptm, N-ANS and N-ANS/N-Me of the cleft palate patients have significant difference （ P 〈 0. 05 ）. Conclusion The patients if receive cleft palate repair have to some degree deficiency in maxillary growth including length, height, width, front and rear position relations. There are no significant differences in the height and position of the mandible bone growth. The data analysis of cephalometry show differences in maxillary height and longi- tudinal position between the repair in the young and in the old, but the younger patients is better than the older at voice quality.
