摘　　要：目的探讨Soehendra取架器在慢性胰腺炎胰管狭窄患者中的应用价值。方法2010年1月至2012年10月沈阳军区总医院内镜科对胰管狭窄患者行治疗性内镜下逆行胰胆管造影术（ERCP）时常规先行扩张探条扩张，如扩张探条不能通过狭窄段，则采用Soehendra取架器扩张。结果共治疗297例胰管狭窄患者，其中11例（3．7％）患者扩张探条扩张失败，改用Soehendra取架器扩张狭窄段。11例患者中男性7例，女性4例，年龄31—74岁，平均（53±3）岁。ERCP造影示胰头颈部狭窄9例，胰体狭窄2例，伴有胰腺假性囊肿1例。治疗过程中使用Soehendra取架器扩张胰管狭窄段均获得成功，无相关并发症发生。术后1个月患者的疼痛程度根据视觉模拟量表（VAS）进行评分，从（3．78±0．83）分下降到（1．56±0．73）分，差异具有统计学意义（t=6．03，P=0．00）。结论运用Soehendra取架器扩张胰管狭窄段是安全、有效的。Objective To evaluate the efficacy of Soehendra stent retriever for chronic pancreatitis patients with strictures of pancreatic duct. Methods Between January 2010 and October 2012, chronic pancreatitis patients with strictures of pancreatic duct underwent ERCP in Department of Endoscopy, General Hospital of Shenyang Command, and dilation was first attempted by using a dilating catheter. If the stricture could not be traversed, the Soehendra stent retriever was inserted over the wire to dilate the stricture. Results Two handreds and seven patients were included and 11 （3.7%） patients underwent Soehendra stent retriever dilation after failure of dilating catheter. Among the 11 patients, there were 7 men and 4 women with a mean age of 53 ~ 3 years old （range 31 ~ 74 ）. ERCP showed 9 cases of pancreatic head stricture, 2 cases of pancreatic body stricture, and one patient was complicated with pancreatic pseudocysts. The Soehendra stent retriever was successfully used to dilate the stricture in all patients, and no complication occurred. The VAS pain score was decreased from 3.78 ± 0.83 to 1.56 ±0.73 （t = 6.03, P = 0.00）. Conclusions Soehendra stent retriever is safe and effective for nancreatie duct strictures dilation.
