颈动脉弹性对短暂性脑缺血发作患者预后影响的临床研究

摘  要:目的应用血管回声跟踪(ET)技术定量监测短暂性脑缺血(TIA)发作患者颈动脉弹性,探讨动脉弹性与TIA治疗后转归的关系。方法选择初发TIA患者86例,应用ET技术实时跟踪描记颈总动脉管壁运动轨迹自动得出压力应变弹性系数(Eρ)、僵硬度参数(β)、动脉顺应性(AC)等参数值,并于治疗后1个月复查动脉弹性。将治疗前后各参数值与健康人群颈动脉正常参数值对比分析。结果 86例TIA患者,治疗后治愈45例(治愈组),梗死41例(梗死组)。TIA患者治疗前,β和Eρ较正常对照组高,差异有统计学意义(P﹤0.05),AC比较差异无统计学意义。治疗后,治愈组β和Eρ较治疗前明显降低,AC升高,差异有统计学意义(P﹤0.05)。梗死组治疗前后两侧颈动脉β、Eρ和AC值比较差异无统计学意义。治愈组和梗死组△β%、△Eρ%、△AC%比较差异均有统计学意义(P﹤0.05)。结论 ET技术可无创量化监测颈动脉弹性功能,对TIA患者的预后转归有早期预测的临床价值。Objective To evaluate the feasibility of using the elasticity of carotid artery through echo-tracking (ET) technique to assess the prognosis of transient ischemic attack (TIA). Methods Eighty-six patients with initial TIA onset were enrolled in the study. The carotid arteries were scanned using a high frequency linear array transducer. The motion tracks of the common carotid artery wall were recorded using ET technique. Parameters of pressure strain elastic modulus(Ep), stiffness (β) and arterial compliance (AC) of TIA patients were calculated before and after treatment respectively, and compared each other using the healthy adults parameters as control. Results After treatment, 86 TIA patients were divided into cured group(n=45) and infarct group (n=41 ). Before treatment, the parameters of β and Ep of TIA patients were significantly highe than those of the control group (P 〈 0.05). There was no difference in AC. After treatment,parameters of β and Ep of patients in cured group were significantly decreased, AC was significantly increased (P 〈 0.05).There was no significant difference of β, Ep and AC in infarct group before and after treatment.While there was significant difference of △β%、△Ep%、△AC% between cured group and infarct group (P 〈 0.05). Conclusion ET technique can be applied to investigate the arterial elasticity directly quantitatively and non-invasively. It is potentially to be used to evaluate the effect of treatment and early prognosis of TIA.
