胃癌全胃切除术后不同消化道重建术式的疗效对比分析

摘　　要：目的 比较分析P型空肠袢食管空肠Roux-en-Y吻合术与改良空肠间置代胃术在胃癌全胃切除术后的疗效.方法 将2008年6月~2012年6月入住我院行P型空肠袢食管空肠Roux-en-Y吻合术的32例胃癌患者设立为对照组,将同期入住我院行改良空肠间Roux-en-Y置代胃术的32例胃癌患者设立为观察组,比较两组胃癌患者消化道重建时间及术后进食、术后6个月体质量下降情况、两组术后并发症情况.结果 观察组消化道重建时间明显短于对照组,观察组患者术后进食情况优于对照组,观察组术后6个月体质量下降幅度明显低于对照组,差异具有统计学意义（P ＜ 0.05）.术后6个月对两组术后并发症情况进行比较观察组术后并发症的发生率明显低于对照组,差异具有统计学意义（P ＜ 0.05）.结论 改良空肠间置代胃术的疗效明显优于P型空肠袢食管空肠Roux-en-Y吻合术,具有进食快、并发症少等优点,值得推广和应用.Objective To compare the efficacy of P-type jejunal loop esophageal jejunal Roux-en-Y anastomosis and improved jejunal interposition on behalf of gastric surgery in gastric cancer. Methods From June 2008 to June 2012 stay in our hospital of P-type jejunal loop esophageal j cancer was set up for the decline in the contml group, B tients with gastric cancer of the stomach surgery set up with gastric gastrointestinal reconstruction time and after ejunum Roux-en-Y anastomosis in 32 patients with gastric y improvement jejunum Roux-en-Y set on behalf of 32 pa- for the observation group, compared two groups of patients eating, after 6 months body weight, and postoperative corn- plications. Results The reconstruction of the digestive tract time was significantly shorter than the control group, the observation group patients eat was better than the control group, the observation group after 6 months body weight de- creased significantly lower than the control group, the difference was statistically significant （P 〈 0.05）. After 6 months of postoperative complications, the incidence of postoperative complications in comparing the observed group was sig- nificantly lower than the control group. Conclasion The improved jejunal interposition on behalf of the efficacy of gas- tric surgery is superior to the P-type jejunal loop esophageal jejunal Roux-en-Y anastomosis, fast eating fewer com- plications, and worthy of promotion and application.
