球型肺结核与周围型肺癌的CT征象对比研究

摘　　要：目的比较球型肺结核与周围型肺癌的CT征象,为临床鉴别诊断提供依据。方法选择2008～2012年肺内球形占位的患者100例作为研究对象,其中肺癌54例,肺结核46例,所有患者均经CT检测,有完整的CT结果,回顾性分析100例患者的CT影像学结果,比较两组的影像学特征。结果周围型肺癌分叶征、深分叶征、毛刺、棘状突起、空泡征、胸膜凹陷征、支气管征显著高于球形肺结核组,而球形肺结核患者空洞、钙化、卫星灶显著高于周围型肺癌患者。结论周围型肺癌和球形肺结核在CT影像上虽然有相似之处,但也有各自的特征,当出现毛刺和深分叶时,应考虑肺癌的可能性大,出现钙化和空洞的时候考虑肺结核的可能性大。Objective To study difference of CT image of ball tuberculosis and peripheral lung cancer. Methods One hundred cases with lungs spherical occupied in 2008-2012 were selected, with lung cancer 54 cases,tuberculosis 46 cases. Retrospective analyzed image date of 100 cases. Results Cases with lobulation,deep lobulation, burr, spiculate protuberance, bronchus encapsulated air sign, pleural indentation, bronchus sign in peripheral lung cancer were more than ball tuberculosis. But cases with cavity,calcification and satellite opacities in ball tuberculosis were more than peripheral lung cancer. Conclusion Image data of ball tuberculosis and peripheral lung cancer has similarity, but has different characters too. When image shows deep lobulation or burr, the possibility of cancer is high. When image shows cavity and calcification, the possibility of tuberculosis is high.
