高频彩超提高脑梗塞颈动脉异常的检出率

摘　　要：目的 利用10MHz高频彩超研究及梗塞患者的颈动脉粥样硬化发生率及病变程度,以及药物治疗的前后对比观察。方法 观察106例脑梗塞患者和50例非脑梗塞对照组的颅外颈动脉粥样硬化程度,并对比12例脑梗塞患者颈动脉病灶中药治疗前后声像图的改变。结果 脑梗塞组内膜至中层增厚的发生率为86.8％,对照组18％(P<0.01),颈动脉粥样斑块的脑梗塞组发生率为77.3％,对照组18％(P<0.01),脑梗塞组颈脉狭窄>50％占6％,颈动脉内径扩大,“S”形扭曲,搏动幅度增加,占17.7％。而对照组无一例出现此现象。颈动脉流速梗塞组明显低于对照组(P<0.01),且伴颈内动脉波形异常P2<P1占22.6％。12例患者经中药治疗一个月对比,IMT从1.45mm降至1.25mm(P<0.01)。结论 高频彩超明显提高脑梗塞患者颈动脉异常的检出率,脑梗塞与颈动脉病变有密切关系,提供了多项有价值指标,且对无症状期动脉粥样硬化患者的早期预防,以及药物治疗颈动脉粥样硬化效果的动态观察,具重要临床价值。Objective To assess the prevalence and severity of carotid arterioscleosis and observe the dynamic chang of the arteriosclerotic plaque in the course of medical treatment with 10MHz high frequency ultrasound. Methods 106 patients with cerebral stroke and 50 control subjects were examined for evaluating the extracranial carotid atherosclerosis, 12 patients with arteriosclerotic plaque were treated by medicinal herbs. Results The prevalence of thickening intima of carotid artery in stroke patients and control subjects were 86.8 % and 18 % respectively (p < 0.01), The rate of carotid plaque in stroke patients and control subjects were 77.3%and18%respectively(p < 0.01) .The most incidence of carotid stenosis > 50% was 6%in the patinents . It was 17. 1%in the patients that carotid shows dilated in 's 'shape distortion and increasing pulsatile degree.The IMT of the plaque in 12 patients was changed from 1.45mm to 1.25mm after 1 month threatment of herbs. Conclusion High frequency ultrasound can improve carotid arteriosclerosis detecting rate. There is a high prevlence of extracranial carotid atherosclerosis in Chinese cerebral stroke patients. Carotid artery sonographic examination can predict the possible source of cerebral stoke in patients suffering generalized arteriosclerosis with plaque in the artery and observe the dynamic change of the plaque after medical therapy.
