益气活血法降低D-Dimer、HSP70高表达对改善AIS临床症状的作用研究

摘　　要：目的:探讨益气活血法降低D-Dimer、HSP70高表达对急性缺血性脑卒中(AIS)临床症状的改善作用。方法:将46例AIS受试者随机分为两组,对照组23例予西医常规治疗,治疗组23例在西医常规治疗基础上加用黄芪注射液合灯盏细辛注射液;疗程5d。比较两组的总有效率及两组治疗前、治疗后不同时点(第1、3、5天)D-Dimer、HSP70、美国国立卫生研究院卒中量表(NIH Stroke Scale,NIHSS)评分、中风证候积分的差别。结果:①两组治疗总有效率比较,差异无统计学意义(P>0.05)。②NIHSS评分:对照组治疗后第3、5天评分与治疗前比较,差异有统计学意义(P<0.05);治疗组治疗后第1、3、5天评分与治疗前比较,差异有统计学意义(P<0.05);两组治疗后第3天比较,差异有统计学意义(P<0.05)。③中医证候积分:对照组治疗后第3、5天评分与治疗前比较,差异有统计学意义(P<0.05);治疗组治疗后第1、3、5天评分与治疗前比较,差异均有统计学意义(P<0.05);两组治疗后第1、3、5天比较,差异均有统计学意义(P<0.01)。④微循环指标:各组内治疗前后D-Dimer、HSP70比较,差异有统计学意义(P<0.05);治疗组治疗后D-Dimer、HSP70的改善优于对照组,差异有统计学意义(P<0.05)。结论:在西医常规治疗基础上联合应用益气活血法,可明显降低D-Dimer、HSP70高表达,改善AIS微循环障碍并促进神经功能缺损的修复,从而显著降低AIS患者NIHSS评分与中医证候积分,改善患者临床症状。Objective To investigate the method of supplementing Qi and activating blood circulation to reduce high expression of D-Dimer and HSP70, and further improve the clinical syndrome of acute ischemicstroke （AIS）. Methods 46 subjects with AIS were randomly divided into two groups, 23 patients in control group were treated with routine western medicine, the other 23 patients in the experience group were treated wit routine western medicine therapy, additional astragalus injection and breviscapine injection for 5 days. The efficiaacy, D-Dimer, HSP70, NIHSS score and syndrome score were compared between the two groups. Results There was no significant difference of efficiency between two groups after treatment（P〉 0.05）. Compared with prior treatment, NIHSS scores in 3d and 5d in control group significantly reduced （P〈 0.05）; NIHSS scores in ld, 3d and 5d in experience group significantly reduced（P 〈 0.05）. NIHSS score in 3d was significant difference between experimental and control groups（P 〈 0.05）. Compared with prior treatment, syndrome scores in 3d and 5d in control group significantly reduced（P 〈 0.05）, and syndrome scores in 1d, 3d and 5d in experience group significantly reduced（P 〈 0.05）. Syndrome scores in 1d, 3d, 5d were significant difference between experimental and control groups（P 〈 0.05）. Compared with prior treatment, D-Dimer and HSP70 in two groups sigrificantly reduced after treatment（P 〈 0.05）, and the experience group decreased more significantly than the control group （P〈 0.05）. Conclusion For AIS patients, Combination of routine treatment and Supplementing Qi and activating blood circulation method could obviously decrease high expression of D-Dimer and HSP70, improve microcirculation, promote the repair of nerve function defect, reduce NIHSS score and syndrome score, improve the dinical symptom.
