颊脂垫瓣填塞腭裂松弛切口对上颌骨发育的临床研究

摘　　要：目的：观察颊脂垫瓣填塞腭裂松弛切口对上颌骨发育的影响。方法：选择30例3～4岁（Ⅱ°～Ⅲ°）腭裂患儿,~应用腭裂常规术式进行修补,其中15例术中即刻游离两侧颊脂垫瓣填塞于双侧松驰切口骨组织与黏膜组织暴露面中,定为实验组;另15例采用松弛切口处碘仿纱条填塞固定,定为对照组;随访观察5年,两组分别取X线头影测量和上牙颌模型,进行测量分析。结果：两组腭裂患儿之间的上颌骨发育有明显差异（P〈0.05）。结论：颊脂垫瓣填塞腭裂松弛切口为上颌骨正常发育提供了良好条件。Objective To observe the effect of using buccal fat pad packing cleft palate relaxation incision on maxillary development. Methods 30 patients aged 3 -4 with complete or incomplete cleft palate were divided into two groups, the experimental group and the control group, each 15 cases. After repairing with routine operation of cleft palate, buccal fat pads of experimental group obtained during operation were employed for filling and covering by suture on bilateral raw bony surfaces. Relaxation incisions of the control group were packed and fixed with iodo form gauze. Two groups were both followed up for 5 years. The X-ray cephalometry and dent）gnathic model of two groups children were obtained and compared. Results There was a significant difference of the maxillary development between two groups children with cleft palate （P〈 0.05）. Conclusion Buccal fat pad packing cleft palate relaxation incision provided a good condition for the development of maxillary.
