手术和LEEP锥切治疗宫颈原位癌的临床疗效分析

摘　　要：目的:观察子宫颈原位癌子宫全切除术和LEEP刀锥切术的临床疗效。方法:对2001年1月—2009年12月子宫颈原位癌的子宫全切除术327例和LEEP刀锥切术88例患者的临床资料和随访结果进行统计分析。结果:3,5,8年总生存率分别为99.28%(412/415),96.50%(303/314),93.48%(172/184)。子宫全切术3,5,8年生存率分别为99.39%(325/327),97.67%(252/258),95.27%(141/148);LEEP刀锥切术3,5,8年生存率分别为98.86%(87/88),91.07%(51/56),86.11%(31/36)。死亡26例中意外死亡1例,余均为心脑血管疾病死亡,无癌死亡病例。两种术式比较,5,8年生存率差异有统计学意义(P<0.05)。结论:子宫全切术和LEEP刀锥切术的临床疗效在短期内无明显差异。对年轻有生育要求者可行LEEP刀锥切术保守治疗,术后严密随访。对无生育要求者应以全子宫切除术为常规术式,同时切除阴道上段1～2cm为宜。Objective：To observate the clinical effect of hysterectomy and high frequency radiosurgical knife （LEEP） cone cutting surgery for cervical carcinoma in situ. Methods：To analyze the clinical data and follow-up re- suits of 327 cases by hysterectomy and 88 cases by LEEP cone cutting surgery from January in 2001 to December in 2 0 0 9. Results ： The overall survival rate of 3,5,8 year was 9 9. 2 8 % （ 412 / 41 5 ） , 9 6. 5 0% （ 3 0 3 / 314 ） , 9 3.4 8 （172/184）. The overall survival rate of 3,5,8 year in hysterectomy was 99. 39%（325/327）,97.67%（252/258） and 95.27% （141/148）. The overall survival rate of 3,5,8 year in hysterectomy LEEP cone cutting surgery was 98.86 （87/88）, 91.07 % （51/56） and 86.11% （31/36）. More than 26 cases died, 1 case was accidental death,died for disease of heart head blood-vessel,cancer deaths. Two kinds of operative methods,the survival rate of 5,8 years has statisti- cally significant （P〈O. 05）. Conclusion：The curative effect of hysterectomy and LEEP cone cutting surgery in the short term have no obvious difference. LEEP cone cutting surgery for young people,have fertility requirements,post- operative close follow-up. Hysterectomy for no fertility requirements cases,the resection of vaginal section is 1~ 2 cm.
