溃疡性结肠炎患者外周血单核细胞中Toll样受体4、NF-κB的表达及其临床意义

摘　　要：目的分析溃疡性结肠炎(UC)患者外周血单核细胞中Toll样受体4(TLR4)、核因子-κ,B(NF-κB)水平的变化,探讨两者在UC发病中作用。方法根据结肠镜检和病理活检结果,106例UC患者分为活动期组(57例)和缓解期组(49例),另同期健康体检患者50例作为对照组,采用实时荧光定量聚合酶链反应(RT-PCR)技术,测定所有对象外周血单核细胞中TLR4和NF-κB的表达情况。结果与对照组比较,UC患者外周血单核细胞TLR4、NF-κBmRNA表达水平显著升高,且活动期组与缓解期组间比较差异有统计学意义(P<0.05):按病理分级与Ⅰ级比较,Ⅱ级、Ⅲ级患者外周血单核细胞TLR4、NF-κB mRNA表达水平显著升高,且随着病理分级升高,TLR4、NF-κB mRNA表达依次增加(P<0.05)。UC患者TLR4与NF-κB mRNA表达均呈线性正相关,相关系数r=0.753(P<0.05)。结论UC患者外周血单核细胞中TLR4及NF-κB呈高表达状态,且TLR4的表达与NF-κB的表达密切相关。Objective To investigate the expression of Toll-like Receptor 4 （TLR4） and NF- kappa B （ NF-κB） in pe- ripheral blood monocytes of patients with ulcerative colitis （ UC ） , and explore the role of TLR4 and NF-κB in the onset of UC. Methods Expression of TLR4 and NF-KB in peripheral blood monocytes were detected by the real-time fluorescence quota polymerase chain reaction（ real time PCR）in the groups of active phase group （n = 57 ） , remission group （n = 49 ）and normal controls （ n = 50）. Result The levels of TLR4 and NF- KB in active phase group and remission group were significantly higher than those in normal controls, and there was significantly difference between active phase group and remission group （ P 〈 0. 05 ）. Comparing with grade I,the levels of TLR4 and NF-κB in peripheral b]ood monoeytes in grade II and III increased remark- ably, and the level increased with the pathology grade of UC （ P 〈 0.05 ）. TLR4 and NF- KB was positively correlated in UC pa- tients, with the correlation coefficient 0. 753 （ P 〈 0.05 ）. Conclusion TLR4 and NF- KB in peripheral blood monoeytes of pa- tients with UC is over-expressed,and the expression of TLR4 and NF-κB is positively related.
