奈达铂联合三维适形放疗法对晚期鼻咽癌的临床效果及不良反应分析

摘　　要：目的探讨奈达铂联合三维适形放疗法对晚期鼻咽癌的疗效及不良反应。方法回归性分析2009年1月～2012年1月到我院治疗的90例晚期鼻咽癌患者的临床资料,根据治疗方案不同分为两组,观察组45例采用常规化疗+奈达铂联合后程三维适形放疗法,对照组45例患者采用常规化疗+后程三维适形放疗法,比较两组效果及不良反应。结果观察组有效率为86.67%,对照组有效率为69.77%,观察组明显高于对照组(P<0.05),两组毒副反应以血液毒性反应为主,观察组毒性反应血小板减少和白细胞减少的Ⅲ～Ⅳ级发生率据均高于对照组(P<0.05),观察组口腔黏膜炎和过敏反应Ⅲ～Ⅳ级发生率也明显高于对照组(P<0.05),两组不良反应均耐受。结论奈达铂联合后程三维适形放疗法治疗晚期鼻咽癌的临床效果良好,毒副反应均耐受,值得临床推广。Objective To explore the curative effect and adverse reaction of naida platinum combined with threedimensional conformal radiotherapy for patients with advanced nasopharyngeal carcinoma. Methods 90 cases with advanced nasopharyngeal carcinoma in our hospital for treatment from January 2009 to January 2012, were divided into two groups according to the different treatments,45 cases of the observation group were treated with conventional chemotherapy and naida platinum combined with three-dimensional conformal radiotherapy,45 cases of the control group were treated with conventional chemotherapy and three-dimensional conformal radiotherapy,the effect and adverse reaction of two groups were compared. Results The effective rate of observation group was 86.67%, the effective rate of control group was 69.77%, the effective rate of observation group was significantly higher than that of the control group （P 〈 0.05）, the main adverse reaction of two groups was blood toxicity, the incidence rate of the toxic reaction thrombocytopenia and leukopenia of grade Ⅲ- Ⅳof observation group was higher than that of control group（P 〈 0.05）, oral mucosal inflammation and allergic reaction of grade Ⅲ- Ⅳ of observation group was significantly higher than that of the control group （P 〈 0.05）.Adverse reactions of the two groups were tolerated. Conclusion The curative effect and adverse reaction of naida platinum combined with three-dimensional conformal radiotherapy is good,have tolerance of adverse reaction,and it＇s worthy of clinical promotion.
