卵巢卵泡膜细胞瘤的临床病理及影像分析

摘　　要：摘要：目的观察卵巢卵泡膜细胞瘤的l临床、病理及影像学特点。方法阿顾性分析32例33个经手术病理证实的卵巢卵泡膜细胞瘤的影像资料，并结合临床、病理资料进行分析。结果 （1）绝经后患者13例（40．62％），绝经前患者19例（59．38％）。（2）瞥侧31例，双侧1例。（3）肿瘤直径5～162nlm，平均（73．03±45．86）ｍｍ；类回形或椭圆形25个（75．76％），分叶状8个（2d．24％）。（4）31个边界清楚．2个合并蒂扭转者边界小清。（4）实性或以实性为主肿瘤26个（78．79％），囊实性5个，囊性2个。（5）15例伴行腹水（46．9％）。（6）10例患者行（YI、或MRI增强扫描，其中8例呈渐进性轻度强化或无强化，2例呈明硅强化。（7）9例患者行背景抑制磁共振加权成像（DWIBS）检查，其中7例呈高或明显高信呼，1例呈等或低信号，l例呈明显低信号。结论卵泡膜细胞榴影像表现具有一定特征性，对该病的诊断及鉴别诊断有重要意义。Abstract：Objective To explore the clinical manifestations,pathological features and imaging characteristics of ovarian thecoma. Methods The clinic,pathology and imaging materials of 32 patients with 3？; ovarian thecomas proved pathologically were retrospec- tively analyzed. Results 40.62% of patients were postmenopausal,59. 38% were pro menopausal. 31 cases were unilateral mass,one case bilateral masses. The sizes of tumors were ranged from 5 to 162 mm with a mean of （73.03＋t5.86） mmin diameter; masses showed round or oval（n 25） and lobulated（n= 8） in shape. All the tumors were with well-defined margin other than 2 tumors with torsiou of pedicle. The tumors were solid mass with or without cystic area（n=26）,cystic solid mass（n--5）,cystic mass（n=2）.8 out of 10 cases displayed gradually and mild enhancement on post-contrast CT/MRI, while 2 cases manifested obvious enhancement. 7 of 9 cases on MR diffusion weighted imaging with background suppression（DWIBS） showed hyperintense,one case showed iso- or hypointensc,one case showed obvious hypointense. Conclusion The imaging features of thecoma have certain characteristics and are helpful in the diagnosis and differential diagnosis of ovarian thecoma.
