睾丸卵黄囊瘤的CT诊断与鉴别诊断

摘　　要：摘要：目的探讨睾丸卵黄囊瘤的CT表现。提高卵黄囊瘤的CT诊断符合率。方法回顾性分析16例经手术病理证实的睾丸卵黄囊瘤的临床和CT资料。结果CT检查16例均为单发，9例肿块表现为囊实性。7例肿块表现为实性，增强扫描动脉期9例实性成分呈轻度强化，7例呈明显强化．16例均以周边强化为主，静脉期16例均呈持续渐进性强化，均见条状分隔线影；5例伴有淋巴结转移，其中l例并伴肺转移；16例血清AFP均明显升高。结论CT对睾丸卵黄囊瘤有一定的诊断价值．结合血清AFP升高有助于术前诊断。Objective To study the CT appearances of testicular yolk sac tumor. Methods Clinical and CT data of 16 patients with pathologically proved testicular yolk sac tumor were analyzed retrospectively. Results The tumors appeared as cystic solid（n= 9） and solid{ n= 7） masses. After contrast administration, the solid component of the lesions showed slight enhancement in 9 cases and obvious enhancement in 7 cases. The edges of all the lesions showed obvious enhancement during the arterial phase. The lesions in all the 16 cases were progressive uneven enhancement, with multiple linear sign inside the lesions during the venous phase. 5 cases showed lymphatic metastasis. Among them, 1 case showed pulmonary metastisis. The serum AFP was increased in 16 cases. Con- clusion Combined with patients＇ serum AFP increasing, CT is valuable in diagnosis of testicular yolk sac tumor.
