健脾扶正汤对晚期原发性肝癌患者临床疗效及免疫功能的影响

摘　　要：目的:观察健脾扶正汤对晚期原发性肝癌患者临床疗效及免疫功能的影响。方法:将60例患者随机分成两组,均采用相同的西医药治疗,治疗组患者加用健脾扶正汤。1个月为1疗程,两个疗程评价疗效。观察患者中医证候、肝功能、免疫功能及不良反应等。结果:两组患者中医证候疗效比较,治疗组与对照组总改善率分别为83.3%和60.0%(P<0.05);两组治疗后肝功能(ALT、AST、TBil)指标较治疗前均有明显改善(P<0.05),其中ALT、AST水平治疗组较对照组改善更明显(P<0.05)。免疫功能:治疗组治疗后CD3+、CD4+水平升高,CD8+下降,CD4+/CD8+比值提高,与治疗前比较差异有统计学意义(P<0.05);对照组治疗前后比较差异无统计学意义(P>0.05);两组治疗后比较,CD3+、CD4+及CD4+/CD8+水平差异有统计学意义(P<0.05)。不良反应治疗组明显低于对照组(P<0.05)。结论:健脾扶正汤治疗晚期原发性肝癌,能改善患者中医临床证候,提高机体免疫功能及生活质量,延长生存期。Objective： To observe the influence on the symptom and immune function of advanced primary liver cancer patients treated with Jianpi Fuzheng decoction. Methods： A total of 60 patients were averagely randomized into treatment group and control group , which were all given the same western medical treatment. The treatment group was additionally given Jianpi Fuzheng decoction. A month for a period of treatment , to evaluate the short - term efficacy after two months. Traditional Chinese medicine symptoms , adverse reaction and immune function were observed and compared between the two groups. Results： There was significant difference in the total response rates between the two groups （83.3%, 60.0%, P〈0.05） . The two group had a comparison after treatment on function of liver （ALT, AST, TBil）, which all improved （P〈0.05） , especially in the aspect of ALT and AST , the treatment group had much more significantly improved （P〈0.05） . The ratio of T - lym- phocyte subsets such as CD3^＋, CD4^＋, CIM^＋/CD8^＋ were all increased after treatment （P〈0.05）, while CD8 ~ decreased （P〈0.05） ; comparison between two group showed the difference was statistically significant （P〈0.05） . In contrast, control group after treatment before treatment and comparison , the difference was not statistically significant （P〉0.05）. Comparison of toxic and side effects of the two group showed the effects in the treatment group was significantly lower than the control group （P〈0.05）. Conclusion： Jianpi Fuzheng decoction can relive clinical symptoms, improve immune function , improve quality of life and prolong survival time of patients with advanced primary liver cancer.
