超声乳化白内障吸除联合折叠式人工晶状体植入治疗小瞳孔白内障46例疗效观察

摘　　要：目的探讨超声乳化白内障吸除联合折叠式人工晶状体植入治疗小瞳孔白内障的疗效。方法对46例(55眼)小瞳孔白内障患者实施超声乳化白内障吸除及折叠式人工晶状体植入术,评价术后疗效。结果本组患者术后视力4.6±0.3,与术前的4.1±0.4比较,差异有统计学意义(u=3.97,P<0.05)。术后获得生理性圆或近圆瞳孔,位置基本居中,人工晶状体位置良好。主要并发症:3眼术中出现前房出血;5眼前囊破裂并延及后囊,少量玻璃体残留瞳孔区;6眼术中表现为典型的虹膜松弛综合征(IFIS);9眼术后前房反应较重(++～+++),其中7眼为眼前节毒性反应综合征(TASS),2例为葡萄膜炎复发,均经激素局部治疗得到了有效控制;8眼术后瞳孔直径4mm,对光反射消失。结论超声乳化白内障吸除联合折叠式人工晶状体植入是治疗小瞳孔白内障一种安全、有效的手术方法。Objective To study the efficacy of phacoemulsification with foldable intraocular lens implantation in the treatment of adhesive small pupil. Methods Postoperative evaluation was performed in 46 cataract patients （55 eyes） with adhesive small pupil treated with phacoemulsification and foldable intraocular lens implantation. Results Visual acuity was improved from preoperative 4. 1 ＋ 0. 4 to postoperative 4. 6 ＋ 0. 3. The difference was statistically significant （ u -- 3.97, P 〈0. 05 ）. The postoperative pupils were round or nearly round,centered, and intraocular lens were in good position. Major complications included：hyphema in 3 eyes, anterior capsule rupture and extension to posterior capsule in 5 eyes with a small amount of vitreous residue in pupil, typical intraoperative flop- py iris syndrome （IFIS） in 6 eyes,moderate to severe anterior chamber reaction （＋＋ ~ ＋＋＋） in 9 eyes,toxic anterior segment syn- drome （TASS） in 7 eyes, and recurrence of uveitis in 2 eyes. All the complications were cured by glucocorticoid topical treatment. In 8 eyes,pupil diameters were 4mm, and light reflex disappeared. Conclusions Phacoemulsification with foldable intraocular lens im- plantation is a safe and effective method in the treatment of cataract patients with small pupil and posterior syuechia.
