乳管镜联合乳管造影对T_0期乳腺癌诊断价值分析

摘　　要：目的:探讨乳管镜联合乳管造影在单纯表现为乳头溢液的T0期乳腺癌中的诊断价值。方法:对本院319例乳头溢液患者中经乳管镜及乳管造影发现并确诊为乳腺癌的患者47例,分析其临床特点。结果:319例患者,其中单纯行乳管镜检查204例,乳管镜联合乳管造影检查115例,共发现并经病理确诊为乳腺癌47例。47例患者均行乳管镜检查,其中16例联合乳管造影,单纯乳管镜检查的诊断符合率为52.5%(31/59),乳管镜联合乳管造影检查的诊断符合率为76.2%(16/21),两者比较差异有统计学意义,P=0.04。乳管镜联合乳管造影检查,术前评估病变范围<2.0cm 6例,2.0～5.0cm 9例,>5.0cm 1例;术后病理明确<2.0cm 7例,2.0～5.0cm 8例,>5.0cm 1例。相比术前病变范围评估,差异无统计学意义,P=0.26。切除后残腔内癌残留,单纯乳管镜组14例,乳管镜联合乳管造影组2例。肿瘤侵犯乳头乳晕,单纯乳管镜组3例,乳管镜联合乳管造影组1例。结论:乳管镜结合乳管造影检查,可以提高乳管镜检查乳腺癌的诊断符合率,进一步明确病变性质、部位及范围,为手术操作及完全切除病变组织提供依据,也可为保留乳房手术提供临床指导。OBJECTIVE：To investigate the diagnostic value of fiberopticductoscoPy combined with galactography in T0-stage breast cancer with simple nipple discharge. METHODS： Enrolling 319 patients with nipple discharge in our hos- pital. Forty-seven of them were detected and diagnosed as breast cancer by using fiberoptieductoscopy and galactography. The clinical significance was evaluated. RESULTS： In the 319 patients,204 patients were detected with fiberopticdueto- scopy and 115 patients were detected with galactography. Totally 47 patients were diagnosed as breast cancer. Fiberop- ticductoscopy was conducted in 47 breast cancer patients, 16 of them were conducted fiberopticductoscopy combined with galactography. Post-operative pathology indicated totally early stage cancers. The diagnostic sensitivity of simple fiberop- ticduetoscopy diagnosis was 52.5 % （31/59）. The diagnostic sensitivity of fiberopticductoscopy combined with galactogra- phy was 76.2 % （16/21）. There was no statistically significance between two groups （P=0.04）. Fiberopticductoscopy combined with galactography indicated the lesion range：pre-operative evauation 〈2.0 cm in 6 cases, 2.0-5.0 cm in 9 ca- ses,and 〉5.0 cm in 1 case; post-operative pathological diagnosis 〈2.0 cm in 7 cases, 2.0 - 5.0 cm in 8 cases, and 〉5.0 cm in 1 case. There was no statistically significance between two groups （P value was 0.26,respectively）- For re- section of residual tumor,14 cases in simple fiberopticductoscopy group,and 2 cases in combination group. No case was found lymph node metastasis. CONCLUSIONS： Fiberopticduetoseopy combined with galactography could increase the di- agnostic sensitivity of breast cancer, and further clarify the characteristics of lesion and its location and range. It provide clinical evidence for surgically complete resection and provide clinical guidance for breast-conserving surgery as well.
