摘 要:目的 探讨脑卒中恢复期尿微量蛋白含量及其临床意义。方法 用速率散射法免疫浊度测定 6 8例脑卒中患者 (其中脑梗死 35例 ,脑出血 33例 )和 30例高血压病患者的尿微量白蛋白 (MA)、转铁蛋白 (TRF)、免疫球蛋白G(IgG)、α1微球蛋白 (α1—MG) ,用酶法测定血总胆固醇 (TC)、甘油三酯 (TG)、载脂蛋白A1(apoA1)、载脂蛋白B10 0 (apoB10 0 )及空腹血糖。结果 脑卒中组尿MA、TRF、IgG、α1—MG水平及血TC、TG、apoB10 0 均高于高血压组 ,尤以脑梗死组升高最为明显 (P <0 .0 5 ) ;脑卒中组尿微量蛋白异常率 (2 5 % )高于高血压组 (10 % )。结论 缺血性中风尿微量蛋白增高与高血压分期、血脂水平及脂质代谢障碍的类型有关 ;早期发现脑卒中患者的肾功能损害 ,有助于判断预后及观察治疗效果。Objective To explore urine microproteins level and clinical value in patiens with cerebral apoplexy during convalescence. Methods Detected urine microalbumin(MA),TRF,IgG, α1-MG in 68 patients with cerebral apoplexy (35 cerebral infarction,33 cerebral hemorrhage) and 30 hypertensions by rate nephelometry immunoassay. TC, TG, apoA1,apoB100 and fasting blood-glucose were detected by enzymology assay. Results Urine MA,TRF,IgG, α1-MG level and blood TC,apoB100 level in cerebral apoplexy group were higher than hypertension group, microproteins increasing was significant in cerebral infarction group especially (P<0.05). Abnormal rate of urine microproteins in cerebral apoplexy group (25%) was higher than hypertension group (10%). Conclusion Urine microproteins increasing in ischemic apoplexy is relative to stages of hypertension, serum lipid level and types of lipid dysbolism. If can find renal function damage early in patients with cerebral apoplexy, which will help to judge prognosis and curative effect.
