摘  要:目的探讨胸外心脏按压相关因素与救援者疲劳的关系。方法将5名技能小组护士每周集中1次,按照2010美国心脏协会心肺复苏及心血管急救指南的操作要求,每人每次在心肺复苏人体模型上连续实施心肺复苏3min,操作完毕,打印包括胸外按压总数、满意按压数等测试结果。共实施心肺复苏50人次。结果随着按压时间延长,按压总数与满意按压数显著降低,疲劳发生率显著增多;救援者客观疲劳发生时间上,女性短于男性,取站姿者短于取跪姿者(均P<0.01);当总平均按压频率大于120次/min时,疲劳时间缩短(P<0.01)。结论救援者疲劳的相关因素包括性别、操作姿势、按压频率、时间等,应1~2min更换一次施救人员,以延长救援者疲劳时间,提高心肺复苏质量。Objective To explore the relationship between external-cardiac-compression-related factors and rescuer fatigue. Methods Five experienced nurses from the skill group were summoned once a week to perform continuous cardiopulmonary resuscitation (CPR) for 3 minutes according to the 2010 AHA Guidelines for CPR ~ ECC. After each performance, data like total number of chest compressions, satisfactory compressions, etc, were printed. The 5 nurses performed 50 times of CPR in total. Results The to tal number of chest compressions and number of satisfactory chest compressions decreased progressively during resuscitation, with proportion of fatigue compression increasing significantly (P(0.01 for both). Female subjects developed fatigue significantly soo- ner than male; those who performed CPR in standing position developed fatigue significantly sooner than those in kneeling position (P(0.01 for both). Subjects who performed CPR at a compression rate over 120/minute experienced shorter time to feel fatigue (P%O. 01). Conclusion Rescuer fatigue is associated with genders, positions, compression rates per minute, and lengths of com- pression, etc. During CPR, it is recommended to alternate rescuers every 1 or 2 min so as to improve the quality of cardiopulmonary resuscitation.
