低位直肠癌患者的保肛手术

摘　　要：目的 :从术后复发率、存活率 ,探讨低位直肠癌保肛术式的可行性。方法 :总结 190例距肛缘 7cm内低位直肠癌患者的临床资料 ,其中距肛缘 3～ 5cm 38例 ,5 .1～ 7cm 15 2例 ;DukesA期 6 5例 ,DukesB期 90例 ,DukesC期 35例 ;行Dixon术16 9例 ,Parks术 8例 ,Bacon术 13例。并与同期 413例直肠癌行Miles术患者比较术后局部复发率、存活率。结果 :保肛术局部复发率为 14.7% ,5年和 10年存活率分别为 5 9.5 %和 5 4.7% ;而Miles术则分别为 12 .3%、6 2 .5 %和 5 7.9% ,二者差异均无显著性 (P >0 .0 5 )。结论 :低位直肠癌保肛术的局部复发率和存活率与Miles术比差异均无显著性 ,值得临床推广应用。Objective:To explore the feasibility of anus protective operations for patients with low rectal carcinoma in aspects of its local post operative recurrence rate and survival rate.Methods:The clinical data of 190 cases with low rectal carcinoma whose affected rectum was 7 cm from the edge of anus were collected.Among them 38 cases lowest affected part was 3～5 cm from the edge of anus and 152 cases 5～7 cm from anus.And 65 cases were in Dukes stage A,90 cases Dukes stage B and 35 cases Dukes stage C.One hundred sixty nine cases were treated with Dixon operation,8 with parks operation and 13 with Bacon operation.The local recurrence rate and survival rate of the above operations were compared with those of the Miles operations on 413 patients with rectal carcinoma.Results:The local recurrence rate of anus protective operation and five year,10 year survival rate was 14.7%,59.5% and 54.7% respectively,while those of Miles operations was 12.3%,62.5% and 57.9 % respectively( P >0.05).Conclusions:Compared with Miles operation,the local recurrence rate and survival rate of the anus protective operation for patients with low rectal carcinoma was not great different,but the patients anus could be protected,therefore the method could be widely used in such surgical operation.
