阿德福韦治疗拉米夫啶耐药后肝功能衰竭的临床分析

摘　　要：目的观察阿德福韦酯治疗拉米夫啶耐药后肝功能衰竭的疗效和安全性。方法将50例在拉米夫定治疗过程中出现耐药性的乙型肝炎患者联合使用拉米夫啶和阿德福韦治疗两个月后,停用拉米夫啶,单用阿德福韦治疗。观察50例患者治疗前后ALT、ALB、TBIL、HBeAg转阴率、HBeAg/抗-HBe转换率、HBV-DNA变化。结果治疗后ALT、TBIL、HBV-DNA定量显著性降低,而ALB、HBeAg转阴率、HBeAg/抗-HBe转换率显著性升高。结论联合使用拉米夫啶和阿德福韦治疗两个月后,停用拉米夫啶,单用阿德福韦治疗12个月能减少耐药率的发生,改善肝功能。Objective To evaluate the effieacy and safety of adefovir dipivoxil in treatment of liver failure with lamivudine resistance. Methods To 50 cases of liver failure with lamivudine resistance treat with lamivudine and adefovir after two months, and then adefovir treatment alone.Observed in 50 patients ALT, ALB TBIL HBeAg seroconversion rate, HBeAg / anti-HBe conversion rate, HBV-DNA changes before and after treatment. Results ALT, TBIL, HBV- DNA quantitative treatment significantly decreased, the the ALB, HBeAg seroconversion rate of HBeAg / anti-HBe conversion rate increased significantly. Conclusion Joint after two months of treatment with lamivudine and adefovir, lamivudine was discontinued 12 months of adefovir treatment alone can reduce the rate of occurrence of drug resistance and improve liver function.
