摘　　要：目的　观察细胞表面分化抗原 (CD) 117在各型白血病中的表达及其意义。方法　采用单标或双标直接免疫荧光标记法标记 2 0种细胞表面分化抗原 ,经流式细胞仪测定。对 185例白血病患者骨髓或外周血白血病细胞的免疫分型及CD117的表达进行分析。结果　CD117在急性淋巴细胞白血病 (ALL)中表达率极低 ,仅占 1.5 % ,CD117在急性髓细胞白血病 (AML) M0 ,M1,M2中表达率较高(>85 % ) ;AML M3患者 ,表达率较低。慢性淋巴细胞白血病 (CLL)全部为阴性。慢性髓细胞白血病(CML)慢性期患者CD117阳性细胞 <10 % ,多数加速期、急变期患者CD117阳性细胞 >10 % ,但 <2 0 %。结论　CD117有助于鉴别ALL与AML。CML患者CD117表达 >10 %应警惕加速和急变的可能。Objective To investigate the expression of CD117 on different types of leukemia and its significance.Method Single color or two color direct immunofluorescence staining were used by means of flow cytometry. Results CD117 was positive in only 1.5% of patients with acute lymphoid leukemia (ALL). Over 85% patients with acute myeloid leukemia (AML) M0, M1, M2 cases expressed CD117,whereas the expression of CD117 was low in AML M3 patients. All of the patients with chronic lymphocytic leukemia were CD117 the negative. CD117 was <10% in chronic myeloid leukemia(CML) in chronic phase, but CD117 was>10% and <20% in most of CML patients in accelerateing or myeloid blastic crisis.Conclusions The expression of CD117 is a useful marker for distinguishing myeloid from lymphoid leukemia. When over l0% of leukemic cells expressed CD117 in CML patients, the possibility of transformation to accelerateing or myeloid blastic crisis phase should be suspected.
