腮腺多形性腺瘤外科治疗临床观察

摘　　要：目的探讨并比较腮腺浅叶部分切除术与全切术治疗多形性腺瘤临床效果及安全性。方法选取某院收治腮腺多形性腺瘤患者70例,采用随机数字表法分为浅叶全部切除组(35例)和浅叶部分切除组(35例),分别采用腮腺浅叶全切术和腮腺浅叶部分切除术治疗;比较两组患者手术一期愈合率,术后复发率及并发症发生情况等。结果浅叶部分切除组患者手术一期愈合率明显优于浅叶全部切除组(P﹤0.05);两组患者复发率组间比较差异无统计学意义(P﹥0.05);浅叶部分切除组患者并发症发生率均明显低于浅叶全部切除组(P﹤0.05)。结论相对浅叶全切术,浅叶部分切除术治疗腮腺多形性腺瘤具有手术创伤小,并发症出现风险低等优点,具有临床应用价值。OBJECTIVE To investigate and compare the therapeutic effects and security of regional excision and total excision surgery on pleomorphie adenoma of parotid gland. METHODS 70 patients with pleomorphic adenoma of parotid gland were cho- sen in our hospital and randomly divided into two groups including total excision group with total excision surgery and regional excision group with regional excision surgery; And the healing rate of first-stage, the recurrence rate and the incidence of com- plication of both groups were compared. RESULTS The healing rate of first-stage of regional excision group was significantly better than total excision group （P 〈 0.05）. But there was no statistical difference in the recurrence rate between total excision group and regional excision group （P 〉 0.05 ）. And the incidence of complication of regional excision group was significantly low- er than total excision group （P 〈 0.05）. CONCLUSION Compared with total excision group, regional excision surgery on pleo- morphic adenoma of parotid gland can efficiently reduce the sugery trauma, lower the complication risk, and has the clinical value of applicatiom.
