鼻内翻性乳头状瘤伴双侧鼻息肉的临床分析

摘　　要：目的分析鼻腔内翻性乳头状瘤伴双侧鼻息肉的临床特征,以避免误诊。方法回顾性分析2008年3月-2012年9月期间本科收治的2例鼻腔内翻性乳头状瘤伴双侧鼻息肉患者临床资料,并结合病理检查结果和文献复习探讨本病的临床特征与诊断要点。结果临床资料显示,2例患者均表现为双侧鼻息肉,但一侧新生物明显增多,深部新生物组织质韧、出血较多;病理检验均为双侧鼻腔内翻性乳头状瘤伴鼻息肉。结论对于双侧鼻息肉患者,如术中发现组织质韧、出血较多,应警惕鼻内翻性乳头状瘤伴双侧鼻息肉的可能,术中应做快速病理检查,以避免误诊。Objective To analyze clinical features of the cases with nasal inverting papilloma accompanying accompanying bilateral nasal polyps in order to avoid misdiagnosis in clinical practice. Methods A retrosPective study was carried out in 2 cases treated from March, 2008 to September, 2012 in our Hospital, with a careful analysis on their cilinical data and histopathological results combined with a literature review on this kind of lesion. These two cases were diagnosed as nasal inverting papilloma accompanying bilateral nasal polyps at last on tile basis of histopathological determination, done during and after the operation respectively.Results These 2 patients were diagnosed as bilateral nasal polyps at first based on physical examination. During the operation, it was found that the polyps-like tissue was much aggressive in one side of nasal cavity, with its texture being in more ductility and being prone to bleeding when operating. Allegro frozen section was carried out during tile operation and showed the neoplastic mass holding tile features of nasal inverting papilloma accompanying the present of nasal polyps. Post-operative histopathological assay confirmed such a diagnosis as well. Conclusion For those patients with bilateral nasal polyps, once they were seen with polyps-like tissue in the texture more ductility and being prone to bleeding during the operation, the diagnosis of nasal inverting papilloma accompanying nasal polyps should be considered and immediate allegro frozen section must be done to make a correct pathological conclusion for ideal treatment.
