摘　　要：目的探讨MDCT对胰腺结核的诊断价值。方法选取本院2001年1月—2012年1月期间收治的疑似胰腺结核患者76例,全部进行MDCT检查、二维超声和病理手术检查,回顾性分析比较临床诊断资料。结果病理诊断胰腺结核阳性63例,胰腺结核阴性13例;本组患者MDCT检查胰腺结核阳性64例,胰腺结核阴性12例。MDCT检查胰腺结核敏感度为96.83%,特异度为76.92%,准确度为93.42%;本组患者二维超声检查胰腺结核阳性54例,胰腺结核阴性9例。二维超声检查胰腺结核敏感度为90%,特异度为56.25%,准确度为82.89%;MDCT检查在诊断胰腺结核的敏感度、特异度、准确度上明显优于二维超声检查,并且两种检查方法在诊断胰腺结核的敏感度、特异度、准确度上均具有显著性差异,P<0.05,有统计学意义。结论 MDCT检查在诊断胰腺结核具有较高的敏感度,值得临床推广使用。Objective To evaluate the diagnostic value of MDCT on pancreatic tuberculosis. Methods 76 cases of suspected pancreatic tuberculosis patients select -ed of our hospital in January 2001-2012 in January were admitted, all with MDCT diagnosis, the two-dimensional ultrasound and pathological surgery, retrospective analysis of clinical diagnostic information. Results The pathological diagnosis of 63 cases of pancreatic tuberculosis positive, pancreatic tuberculous negative 13 cases~ 64 cases of pancreatic tuberculosis positive MDCT examination of this group of patients, negative in 12 cases of pancreatic tuberculosis. MDCT examination of pancreatic tuberculosis sensitivity of 96.83% and a specificity of 76.92%, 93.42% accuracy in this group of patients with two-dimensional ultrasound pancreatic tuberculosis was positive in 54 cases, nine cases of pancreatic tuberculosis negative. The two- dimensional ultrasound pancreatic tuberculosis sensitivity 90%, specificity 56.25%, 82.89% accuracy MDCT examination in the diagnosis of pancreatic tuberculosis sensitivity, specificity, and accuracy was significantly better than the two-dimensional ultrasound and two kinds of inspection methods in the diagnosis of pancreatic tuberculosis sensitivity, specificity, and accuracy with a significant difference, P 〈0.05, statistically significant. Conclusion MDCT examination in the diagnosis of pancreatic tuberculosis has a high sensitivity, is worthy of clinical use.
