摘　　要：目的探讨骨桥蛋白(osteopontin,OPN)在乳腺癌中的表达及其与预后的关系。方法回顾性分析2002年1月～2003年12月乳腺癌患者239例。原发灶石蜡标本制作组织芯片,采用免疫组化EnVision法检测OPN的表达。结果 OPN的阳性率为54.39%,其表达与ER(P=0.004)、PR(P=0.008)相关,单因素分析OPN阳性组的无瘤生存低于阴性组,差异有显著性(Log-rank检验,P<0.01),在患者年龄、原发灶大小、组织学分级、病理学分类、腋窝淋巴结状态、ER、PR和人表皮生长因子受体2(human epidermal growth factor receptor 2,HER2)的多因素Cox模型预后分析中,OPN表达、腋窝淋巴结状态、组织学分级是独立的预后因素,其危险比为2.55(95%CI为1.26～5.17)。结论检测OPN表达有助于乳腺癌预后的判断。Purpose To investigate the expression of osteopontin and its prognostic significance in breast cancer. Methods 239 pa- tients of breast cancer admitted between Jan, 2002 and Dec, 2003, were included in the retrospective study, paraffin-embedded speci- mens of the primary tumors were collected and tissue micro-array blocks were constructed. Osteopontin expressions were detected by u- sing immunohistochemistry. Result The positive expression rate of osteopontin was 54. 39%. The osteopontin expression was correla- ted with ER （P = 0. 004） or PR （P = 0. 008 ） expression. Univariate analysis revealed that there was a significant correlation between expression of osteopontin and disease free survival （ Log-rank test, P 〈 0. 01 ）. Cox regression analysis indicated that expression of os- teopontin was an independent prognostic factor in breast cancer （HR = 2. 55 ） （95 % CI = 1.26 - 5.17）. Conclusion The detection of osteopontin expression may be helpful for evaluating the prognosis of breast cancer.
